I, AYLENE SALCEDO DELA CRUZ, of legal age, temperate at all times and in full
possession of my mental powers, hereby declare:
1. That I am the illegitimate mother of the minor JUDE MARCO S. ALMARES, of
seven years old, whose father is now deceased;
2. That I and Proceso Almares Jr., the father of the said minor child has not been legally
married in accordance with law.
3. That I have not, at anytime, abandoned said minor child;
4. That I got married to Dennis A. dela Cruz, and believing that it would be for my
illegitimate sons own interest and benefit, I hereby give my written consent, freely
and voluntarily, that said child be adopted by the us in accordance with law.
