PETITION

In the Matter of Adoption of Jude Marco S. Almares

COMES NOW the petitioners by the undersigned attorney, and unto this Honorable
Court, respectfully avers:
1. That the petitioners are husband and wife, both of legal age, and residents of 919
Banuyo Street, San Antonio Village, Makati City;
2. That they have no legitimate children or descendants and hereby desire to jointly
adopt a minor, named JUDE MARCO S. ALMARES, of seven years old, illegitimate
child of AYLENE S. DELA CRUZ, one of the petitioners.
3. That the father of the said minor is now dead and only the mother named AYLENE S.
DELA CRUZ, one of the petitioners is now married to DENNIS A. DELA CRUZ,
one of the petitioners , is alive;
4. That said AYLENE DELA CRUZ, the mother got married to DENNIS A. DELA
CRUZ, one of the petitioners, is not insane, nor intemperate, nor has she abandoned
such minor, and with full knowledge that it would be for JUDE MARCO S.
ALMAREZ benefit and interest hereby expressly gives her written consent to the
adoption, as evidenced by Annex A, hereto attached and made an integral part of
this petition;
5. That the petitioners are qualified to adopt the said minor, and will be able financially
and morally, to bring up and educate said minor, properly and adequately.
WHEREFORE, it is respectfully prayed that upon due notice and hearing, judgment be
entered and adjudging that the minor child JUDE MARCO S. ALMARES be freed from legal
obligations, of obedience and maintenance with respect to his natural parents, and that he be
declared to all intents and purposes, the child of the herein petitioners, and that his surname be
changed to that of the petitioners.
