CommunityHealth, a volunteer-based FREE health clinic on Chicago's West Side, opened its doors in 1993. The clinic represents a grassroots
response to the enormous and rapidly increasing number of medically uninsured people in Illinois. We are especially unique in that patient care is
rendered by volunteer health professionals - - over 350 volunteers, 120 of whom are physicians, conduct approximately 14,000 pro-bono patient care
visits annually.
Since our doors opened we have provided over 72,500 free patient visits. Today we operate six days a week, including four evenings and Saturdays. Over
85 percent of our patients are return patients who look to us as their primary source of care.
CommunityHealth's patients are extremely vulnerable: All lack health insurance coverage An estimated 80% come from working households, yet all are
poor, at or below 200% of the Federal Poverty Level 18% are children Over 80% are from minority populations, of which 70% are Hispanic Most are new
immigrants and do not speak English Nearly all face educational, linguistic and cultural barriers to health care Many travel great distances to receive care at
CommunityHealth, with over 64% residing in outlying neighborhoods The majority have put off seeking early, medical care because they cannot afford it
or are afraid to seek care in a new surrounding.
It is remarkable that in 2003, with just over 16 'full time equivalent' employees, CommunityHealth:
1)

provided over 14,000 pro-bono patient visits via our standard clinic session format, and an estimated 2,000 additional visits rendered
through our educational programs;

2)

dispensed over $985,684 worth of FREE medications from our dispensary;

3)

conducted 7,560 FREE laboratory procedures; and,

4)

trained over 255 medical students and 170 residents.

More than 90% of CommunityHealth's patient visits are by appointment. Our services include: preventive, primary and illness care; women's health services; clinical
laboratory testing; comprehensive diabetic care; 'take-home' medications and diabetic supplies; school exams and immunizations; social services and
case/referral management; specialty services [cardiology, dermatology, gynecology, nephrology, ophthalmology, pediatrics, podiatry, psychiatry,
physical therapy rheumatology and surgery]; patient education; and, testing, counseling and treatment of sexually transmitted diseases.
Due to lack of accessible alternative sources of care for an estimated 1.3 million uninsured in greater Chicago, our clinic serves as a citywide resource. Please know
too, that CommunityHealth does not provide services to people with Medicaid or Medicare, as these government programs are health insurance programs.
Patients who come to CommunityHealth literally have nowhere else to go.

Fact Sheet
CommunityHealth is a volunteer-based free health clinic that provides basic healthcare to people without health insurance and to persons with a family income
less than 200 percent of the Federal Poverty Level. Dr. Serafino Garella and other concerned individuals founded CommunityHealth in 1993. CommunityHealth
is a unique health clinic in relation to other clinics in Chicago because of two major reasons: CommunityHealth receives no government funding and its primary
work force is volunteers. In 1999, volunteers performed 93% of all patient services.

Over 360 volunteers staff CommunityHealth. Volunteers


perform clerical, interpreting, nursing, over medical, legal, and
social service duties.

CommunityHealth has had over 42,000 visits since the clinic


opened in 1993.

CommunityHealth's patient visits have grown by 500 percent


since it's opening in 1993.

In 2001, CommunityHealth provided nearly 16,000 free patient


visits.

Volunteers are an enormous component of CommunityHealth's success. There are volunteer opportunities
available for everyone. Our volunteers deal with a diverse group of patients. We treat all of our volunteers and
patients with respect and without judgment. CommunityHealth believes that healthcare is a right not a privilege.
Some of our volunteer positions are listed below.
Front Desk Volunteer
Volunteers are needed to assist in patient resignation, answer patent questions, scheduling patient appointments and occasional filing
and clerical support.

Intake Volunteer:
Volunteers are needed to assist in qualifying, screening and scheduling potential patients for services at CommunityHealth.

Clinic Flow Coordinator:


Volunteers needed to assist clinic staff and volunteers with patient flow, coordinating interpreters during clinic sessions, answer clinical volunteer questions, and assist in paperwork
associated with referral to Cook County Hospital. Clinical background is beneficial but not required.

Interpreter:
Volunteers are needed to provide interpretation for patients that do not speak English. Fluent Spanish and Polish speakers are especially needed. Cultural background is a plus.

Lab Volunteer.
Volunteers are needed assist with blood draws, urine collection and sample processing.

Dispensary Volunteer:
Volunteers are needed to assist providers in determining the most appropriate means to provide patients with medications. Medical background is necessary.

Volunteer Intake Nurse:


License nurses needed to interview new patients upon intake to assess urgency of medical attention.

Medication Nurse Volunteer


License nurses needed to educate and review medications with patients when patients pick up their prescription refills.

Triage Volunteer:
Medical students, nursing students, CNA's, medical technicians, LPN's, and RN's are needed to take vitals and patient histories and assist in general clinical operations.

Volunteer Physician:
Licensed physicians needed to provide quality care. Primary/preventative care is the predominate practice; however specialty providers are greatly needed. Ophthalmologist,
cardiologist and psychiatrist are especially needed.

Community Events Volunteer/Young Professionals:


Assist in the coordination and production of special and community events; including the annual gala, volunteer appreciation and Young Professionals events.

Advocate/Escort Volunteer:
Volunteers needed to accompany our patients to Cook County Hospital and advocate for their medical needs and/or provide interpretation for our non-English speaking
patients.

Child Care Volunteer:


Volunteers are needed to supervise and coordinate activities for the childrens waiting/playroom.
