PROCEDURES REGARDING REPORTING OF MEDICAL OR FIRST AID INCIDENTS

BY MEDICAL STAFF
1) The patient reports to the first aid clinic accompanied by contractor or company
representative.
2) Details of each patient should be entered in the medical log file including name,
company, badge number, mobile number, and room number.
3) The nurse on duty informs Fluor Camp HSE&S Supervisor, Bruce W. Kyriakides in case
of an incident in the camp, or informs Primary Case Manager Huey Callegan if the
incident happened in the site.
4) The First Aider examines the patient or requests that patient be examined by the Project
Medical Officer in cases where further treatment may be required.
5) All cases involving the eye will be examined by the Project Doctor.
6) A non-emergency case is issued a Medical Attendance Form (MAF) to be presented at the
Out Patient Department of the hospital.
7) Each company or subcontractor has its own MAF booklet, the serial numbers of which
are stored in the hospitals database which indicates the company it was issued to. Unless
otherwise authorized, the medical attendance forms for a particular company should only
be issued for its own workers.
8) The blue copy of the MAF is given back to the patient when he leaves the hospital. This
copy is valid within 7 days from its date of issue and may be used by the patient for
subsequent consultations.
9) The nurse on duty do not need to inform the medical officer, HSE&S area supervisor, and
primary case manager of routine referrals such as patients who regularly go to the
hospital to claim their monthly supply of medicines, as well as patients who need to go
back to the hospital for subsequent consultations for the same health problem.
10) Emergency cases may be sent to the hospital without a MAF. However, for subsequent
consultations, the patient needs to bring with him a MAF.
11) Emergency cases should be accompanied by a company representative.
12) Transport of patients.

Patients with life-threatening conditions and patients who need to lie down on the
way to the hospital should be transported by a RLMC ambulance.

Passenger vehicles may transport non-emergency cases, provided that the


patients can sit without causing them undue discomfort or without aggravating
their health problem.

In a non-emergency case, the patient shall be transported to and from the hospital
by the Contractor representative or, in the case of a subcontractor, by the main
contractor.

13) A medical service questionnaire should be filled out by the nurse and given to the
medical officer in any of the following:
A work related case that may or may not require treatment at Al Madinah
Health Center or RLMC
A case of questionable cause or reason for patients illness
A case where the patient is asked to return to the clinic later on for further
treatment or further observation.
14) Further updates on patients condition shall be supplied to the case management team by
the contractor or subcontractor.
