Dear PHA Member:
The PHA COMELEC is pleased to announce that the PHA election for the Board of Directors will be on November
17, 2016, Thursday, during the PHA 67th Annual National Convention to be held at The Manila Hotel, Manila, Philippines.

Regular members of Good Standing may nominate Candidate/s for the eleven (11) Board Seats , namely:
Luzon:
4 seats (Private)
3 seats (Government)
Visayas:
1 seat (Private)
1 seat (Government)
Mindanao:
1 seat (Private)
1 seat (Government)

Aside from the enclosed copy and emailed version, you may refer to the PHA Website (http://pha.org.ph) for the
downloadable PHA COMELEC ELECTION RULES for the year 2016. The salient features are:

Nomination shall be done in writing in the nomination form provided herein. Nominations are given by a member
of Good Standing. Self Nomination is not allowed.
Only Authorized Representative is allowed to vote and be voted upon.
Proxy voting by a person other than those working for the member hospital shall not be allowed.
Nominations are accepted not later than 5:00pm of August 26, 2016. Send nominations by courier or mail to: PHA
COMELEC, c/o PHA Secretariat, Philippine Hospital Association, #14 Kamias Road, Quezon City, or email/fax
(philhospitalassn@gmail.com / (02) 921-2219).
The Nominees shall be required to submit the following documents:
a. Acceptance of Nomination (pro forma is hereby attached)
b. Curriculum Vitae with recent 2X2 colored picture.
c. Proof of designation as Authorized Representative Appropriate certification shall be issued by the following:
i. Government Hospitals: Appropriate appointing authority from the local Government of Legislative Board
or the Department of Health, as the case maybe, or any legal authority with direct supervision and control
over the position.
ii. Corporations: Corporate Secretarys Certificate indicating there in the minutes of the Board Meeting or
Board Resolution; or appointment letter from the CEO or Chairman of the Board
iii. Private: Single Proprietorship: Letter of affirmation as the Hospital Owner and representative or
certificate from the hospital owner or ascendant authority.
d. Certificate of Good Standing from the PHA Secretariat
e. Optional: A short narrative of candidates Plans and Programs when elected to the PHA Board.
THE PHA COMELEC, after receiving the nominations, shall meet and evaluate the Nominations. The Nominees shall
immediately be notified by phone calls/emails but the official notice shall be sent in writing by special courier not
later than September 10, 2016.
The Acceptance of Nomination and submission of Documentary requirements by nominees shall be received by
the PHA COMELEC not later than 5:00pm of October 10, 2016.
The PHA COMELEC shall immediately meet thereafter to make final evaluation of the acceptances and to prepare
the list of Official Candidates for the PHA Board of Directors.
A copy of this list shall be posted on the website and PHA FB Fan page accounts, emailed and sent to the general
membership on October 10, 2016. The Campaign Period shall start on October 10, 2016 to Election Day.
