GENERAL APPEARANCE: well developed and well nourished,
pleasant, alert and oriented, NAD. HEENT: HEAD:, normocephalic,
EARS:,, tympanic membranes normal, EYES:, eyes normal, PERRL,
NOSE:, nose clear, THROAT:, pharynx and tonsils normal. ORAL
CAVITY: normal, TEETH:, normal dentition, no dental caries.
