of legal age, Filipino and whose designation, signature

and photographs appear below, to be our true and lawful
attorney-in-fact. To us in our names, places and stead, to
collect / receive our regular monthly salaries /
differentials / longevity pay/ allowance and the likes by
virtue of our being public school teachers / clerk / security
guard / janitors in the Division of City Schools, Manila.
It is understood that the school Principal Shall be
primarily accountable for all the checks and released
from the Division Office and that utmost diligence shall be
exercised in the subsequent releasing of claims to the
proper claimants.
That in case of transfer or separation from the
service of the person, this authorization is hereby
terminated and shall be subject for replacement.
