AORTIC STENOSIS

Etiology
a. Age related degenerative
calcific change atau
senile aortic stenosis
b. Congenital 1-2%
populasi dengan abnormal
bicuspid valve
c. Rheumatic valve disease
Pathology
a. Degenerative AS terlihat
kalsifikasi katup. Jika
terjadi atherosclerosis, terlihat proliferasi selular, inflamasi, akumulasi lipid,
dan peningkatan marginasi makrofag dan limfosit T
b. Congenital terlihat deposisi kalsium akibat aliran turbulen darah yang
mengenai katup
c. Rheumatic AS fibrosis jantung dan fusi commisurenya serta pembentukan
calcified mass
Pathophysiology

Clinical maifestation and evaluation

o Symptom utama angina, syncope, dan heart failure
o Hasil pemeriksaan fisik :

Coarse-late peaking systolic ejection murmur

Pulsasi carotid artery yang melemah (parvus) dan terlambat
(tardus)
Adanya S4 karena atrial contraction
Berkurangnya suara S2
Natural history
Prognosis cukup buruk. Data dari Mayo Clinic menunjukkan bahwa 1-year surival
rate hanya 57%
Treatment
o Aortic valve replacement (AVR)
o Berhati-hati menggunakan obat-obatan yang menyebabkan hipotensi
seperti vasodilator, diuretik, dan nitroglycerin

AORTIC REGURGITATION

Etiology
a. Abnormalities of valve leaflets
Congenital (bicuspid valve)
Endocarditis
Rheumatic
b. Dilatation of aortic root
Aortic aneurism (inflamasi;
connective tissue disease e.g.
Marfan Syndrome)
Aortic dissection
Annuloaortic ectasia
Syphillis
Pathophysiology

Clinical

Manifestation & Assesment

o Common symptoms dyspnea on exertion, fatigue, dcrease exercise
tolerance, dan sensasi tidak nyaman akibar high pulse pressure
o Pemeriksaan fisik
a. Bounding pulse
b. Widened pulse pressure
c. Blowing murmur of AR (Austin Flint murmur) akibat turbulensi aliran
darah melalui mitral valve saat diastol
d. Chest radiograph enlarged left ventricular silhouette
Treatment
o Untuk asymptomatic dan symptomatic patients obat-obatan afterloadreducing vasodilators ( ca channel blocker, angiotensin converting enzyme
inhibitor)
o Syptomatic patients, atau asymptomatic patients dengan severe AR
surgical corrections
