XX2081086 of legal age, Filipino citizen, presently residing at Penampang
Proper Donggongon, Penampang Sabah after having been duly sworn to in
accordance with law hereby depose and say:

1. That I am legal mother of CHERYL ANTIPUESTO DINOPOL who

was born on 28 May 1988 at LOPEZ JAENA, MISAMIS
OCCIDENTAL.

2. That as mother of CHERYL ANTIPUESTO DINOPOL I hereby

certify that my daughter is still single and has never been married.

3. That there is on legal impediment on her intended marriage with Mr.

SHIM YUNG VUI @ SIM YUNG VUI, I/C No. 650416-12-5427 and
I hereby give her my parental blessing to marry said person.

04. That I am executing this affidavit to attest the foregoing facts in

connection with my daughters application for a marriage in Kuala
