The HSA is administered by Empire. More details will be provided to you from Empire directly.
All other plans are offered through AMERIFLEX. When it comes time to get reimbursement from your AMERIFLEX
account, it is simple. When you enroll, you will receive the AMERIFLEX Convenience Card. This debit card works similarly
to your regular bank debit card because it provides electronic access to the funds in your tax-advantaged savings accounts.
You can use the same card for all the plans you are enrolled in. Simply present your card to your provider or for eligible
commuter expenses. When your card is swiped, the system qualifies the expense to ensure that the expense and provider
or merchant are eligible under your plan, and then your claim is paid, up to the balance in your account.
You can also set up the AMERIFLEX member online account manager at www.flex125.com. Go to the Member drop-down
and select the Member Portal Login link. You can register under the login box on the left side of your screen by clicking on

Hospital Indemnity Insurance: This option pays a flat dollar amount if you are hospitalized for invasive diagnostic exams,
surgery, or childbirth.
Specified Disease Insurance (Cancer): This option pays a flat dollar amount if you are diagnosed with cancer to help you
manage your expenses while you are getting well.
Accident Indemnity Advantage: This option pays a flat dollar amount if you experience an unexpected impact that leads
to broken bones, burns, concussion, laceration, or other related injuries.

If you are a regular, full-time employee, you may choose to purchase Critical Illness Insurance through UnitedHealthcare.
You must elect coverage for yourself in order to elect coverage for your spouse and/or child(ren). This plan provides another
form of financial protection if you or an eligible family member becomes critically ill; examples include cancer, heart attack,
stroke, coma, permanent paralysis, severe burns, or major organ failure.
