GP PRACTICE BOUNDARY Broughty Family Healthcare

NHS Tayside has received an application from Broughty Family Healthcare, 103 Brown Street,
Broughty Ferry, Dundee DD5 1EP, to amend their practice boundary.

As part of the Boards consultation process, we would like to invite you to comment on the
attached proposal. A map setting out the proposed new boundary and that of the existing
boundary is enclosed. All comments received will be considered via the General Practice Advisory
Committee. In order that your comments can be given full consideration, I would be grateful to
receive your response by no later than 27th January 2017.

I have listed below local practices whose boundaries include areas which the practice wishes to
exclude in their proposed boundary. The practices are

You may care to note that the practice has been reminded that patients who live within their
previously agreed boundary must not be removed from their list as a result of any variation. Under
contractual requirement, these patients must remain registered until such time as they either de-
register naturally through the passage of time or move to an address out with the practice
boundary.

However you may also care to note that, in these circumstances there is nothing to preclude a
practice writing to its patients to explain the boundary change and ask if they would be prepared to
re-register with a local practice whose boundary covers the area where they reside.
