AFFIDAVIT
I, EVELYN R. PERALTA, of legal age, married, Filipino, and a
resident of Busilelao, Echague, Isabela, Philippines, after being
sworn in accordance with law, hereby depose and state

1. I am married to Joey P. Julian on March 30, 2008 before

Rev. Salvador T. Avarientos, Church Minister of International
Baptist Missionary Fellowship Philippines. A copy of our
Certificate of Marriage is hereto attached and marked as ANNEX
A;

2. However, in the VOTERS REGISTRATION issued by the

Commission on Election (COMELEC), my civil status still appears
to be single.

3. That I am still single when I applied for registration at the

COMELEC.

4. I attest to the truth of the foregoing statements to the

best of my own personal knowledge and based on authentic
documents or records. I am executing this affidavit to support my
application abroad as well as for all legal intents and purposes it
may serve.
