AFFIDAVIT OF UNDERTAKING

I, Nora L. Bacar of legal age, Filipino and a resident of Purok 6,

Kinamaligan, Masbate City after having been duly sworn to in accordance
with law, hereby depose and say:

That I am a bona fide employee of Provincial Treasurers Office of

Masbate.

That due to the implementation of Memorandum Circular No.859 of the

Bangko Sentral ng Pilipinas, all ATM card holders are to replace the old
magnetic card with an EMV chip card to prevent Card Fraud and other
Skimming Acts.

Further, I commit not to declare its loss in any event for purposes of
defrauding or misleading my creditor ARB Lending Investor, Inc. because
ARB learned that I have previously sought for a replacement for lost ATM
while my ATM was used as a collateral for a loan with another creditor.

That I have executed this affidavit to attest to the truth of all the
foregoing and satisfy the inquiry or requirements of any person or office
particularly the Development Bank of the Philippines or any Government
instrumentality.
