    account with Unique Client Code allotted to me/us by RKSV, situated at
    212 Trade Centre, BKC, Bandra (E), Mumbai 400 098. In this regard, you are requested to
    reactivate my/our trading account and allow trading with the immediate effect.

    I/we hereby confirm and undertake that :

    1. I/we have completed all the KYC formalities and submitted all the required documents
    at the time of opening the trading account originally.

    2. There is no material changes in the information provided to you in KYC form.

    3. A duly filled up, signed & stamped data updation form is enclosed with latest changes
    required to be done in my UCC a/c with you.
