Rating Justification
When doing my vital signs on the patient I accurately took vital signs. When giving the temperature I had trouble getting the oral temperature. The probe kept falling out of the
patients mouth. I was told by a visitor of the patient it might be better to hold the probe in place to get the temperature. I thanked her for advice and it worked much better.
Properly assess an individual jaw strength when taking the temperature. Also, to make things easier with elderly patients I should always hold the temperature probe in place.

Rating Justification
I was able to reach a proper therapeutic relationship with my patient. This included properly explaining to the patient what I was doing at all times. The patient seemed comfortable of
my practice. With my patient I went into my personal life when talking. This included talking about my marriage status, and why I was pursuing nursing.

Rating Justification
When removing a saline lock, I must use proper techniques to make the experience more comfortable for my patients. This includes proper removal of tape around the incision, and
showing the patient where to hold the gauze to decrease bleeding and bruising.

When moving my patient from the bed to the chair, I did not realize that the chairs leg rest had to be raised. At first I didnt even realize it could be raised. With a hip replacement it is
important to have the leg raised to ensure proper healing.
I will make sure to raise the leg rest in the chair to make the patient more comfortable. I will search for the hydraulic lever and make sure I elevate the legs for my patient who have the
hip replacement.

Rating Justification
When walking outside my patients room, they had an emergency where she needed a bed pan. This was not my patient but I rushed to get her a bedpan. I was not in time. She was
embarrassed and ashamed what she had done. I ensured a professional attitude and made sure I would help her. Shortly after two other student nurses came in to help in which they
were assigned to her as a patient. I relinquished my duties and the other two students took over.
Make sure to know which nurses and students are assigned to which patient. In this case I could pass on the information to them effectively.
