I, Savan Narayanbhai Patel, son of Narayanbhai Ambalal Patel, presently residing at

3-18-85, Narayan Patel Pole, Visnagar, Mehsana, Gujarat - 384315 mobile number
+91 8460038625, email address sn.patel1993@yahoo.in solemnly on oath state
that:-

a. That the choice of this course Master of Information Systems at University of

Southern Queensland is entirely made by me after fully understanding the
course content and finding out details from internet, browsing brochures and
catalogs.
b. That of all the Education provider offering this course the above named
Education provider suits me the best
c. That the fee for the entire program is AUD 47360 and living expenses in
Australia are around AUD 19830 per year.
d. That I have understood that this program is on self funding basis and the fee
for the course is to be paid at the commencement of each of the semesters.
Further I understood that this is not Scholarship, Employment or Immigration
based program.
e. I agree to pay the first semester / enrolment fee of AUD 15536 directly to the
Education provider.
f. I affirm that all the documents including academic documents and financial
documents to be submitted to the Education provider and to the concern High
Commission are genuine and are procured from the original source directly,
which are further substantiated as they are duly notarized by authorized
notory. That it is clarified that Velox Overseas Education only act as free
Counselors are no way involved directly/indirectly for procuring or submission
of any documents.
g. I state that I am solely responsible for production of any tampered or
incorrect or fraudulent document and be answerable to any query arising
thereof. That in case if found that any of the document is tampered or
fraudulent or managed or incorrect including any narration made in the form
or any document then I undertake that I shall be solely responsible for the
consequences arising thereof and am aware that I can be sued/prosecuted
not limited to criminal proceedings for my unwarranted act at my own costs
as the same is necessitated due to my own cause.
h. I state that the certificates and mark sheets are awarded to me by Gujarat
Secondary and Higher Secondary Education Board and Gujarat Technological
University.
i. I say and state that I have agreed to pay the tuition fees and/or the initial
deposit to the Education provider and/or charges applicable for admission of
student visa, which I have understood are non refundable in case of
fraudulent documents submission.
 j. I further say that Velox Overseas Education is only a free Education
counseling provider which provides free Education Counseling and therefore I
indemnify Velox Overseas Education against any of past/present/future acts.
k. I say that I have been appraised and I have agreed of my own will that Velox
Overseas Education provides with free Education Counseling and has no
responsibility what so ever for obtaining my visas, jobs, residence, citizenship,
work permits during and after your course of education.
l. I say that it has been mutually discussed between me and Velox Overseas
Education regarding the courses, tuition fees, Education provider, boarding
and lodging expenses, location of Education provider, Australias working
conditions, etc and I have consented to Velox Overseas Education that I shall
not change the Education provider at least before completion of one academic
year.
m. I say that my sponsors are genuine and are keen to sponsor my further
studies. The information, documents and funds provided from my sponsors
are from original source and will be used only for my further overseas
education purposes.
n. I declare that I am married. I and my wife have also never applied for any
kind of visa to any country.
o. I say that I have been appraised and I have agreed that Velox Overseas
Education act only as free Counselors and is no way responsible for the
following causes mentioned hereunder;
1. In event of cancellation of program by the Education provider at the
last minute.
2. For the refund of tuition fees or excess payments from the Education
provider.
3. Accommodation expenses.
4. Internship jobs during and after studies.
5. For any actions of the Education provider.
6. For any change in the countrys policy.
7. For change in the education provider policy/rules/fees etc.
8. For loss of passport of any other documents of yours during the transit
by the courier company.
9. For bankruptcy/liquidation of the Education provider.
10. In event of suspension or withdrawal of license of Education provider
by the concerned authorities/Government of the respective country.
11. For any undisclosed information/fraud/misleading information related
to my past history.
p. I agree to the fact that my deposit / tuition fees paid to the education
provider will not be refunded in any fraud in my documentation and
information is detected.

In case of any dispute Ahmedabad would be the legal jurisdiction and will be
considered as the place, where the cause of action shall be deemed to have arisen.
