Proposal for Delivering Case Management Services

ABC AGENCY intends to implement a Wraparound System
of care, shown to be highly effective in promoting excellence
in service delivery while promoting positive system of care
changes for populations served, through provision of high
quality coordinated case management support services
maximizing organizational and natural (formal and informal)
supports for participants of the POWER Program. A
nationally acclaimed consultant for the Wraparound process
will provide training and guidance through collaborative
efforts related to other ABC AGENCY projects. In the event
ABC AGENCY shares case management responsibilities
with other POWER partners, ABC AGENCY will invite
partners to Wraparound trainings provided by the national
consultant. The Wraparound process will be driven by the
following ten key principles: Participant and Family Voice
and Choice; Natural Supports; Individualized Service;
Strengths Based; Persistence; Cultural Competency;
Team Based; Collaboration and Mutual Commitment to
Team Goals; Community Based; and Outcome Based.
The training and implementation of the Wraparound process
for POWER participants will be a novel approach to helping
POWER participants and their support system to reach their
life goals. ABC AGENCY desires to expand the model for
case management services previously provided to help
ensure participants are more fully engaged, supported and
positively transitioned during their recovery journey resulting
in not only sobriety but the realization of their personal and
family goals resulting in the highest possible quality of life
throughout the challenging process of recovery. The end
result of the process will be the participants integration in to
the Recovery Community that will sustain the person without
services and supports from the POWER Program.
ABC AGENCY believes given the complexity of the co-
occurring nature of the people referred to the POWER
program a combination of Master level trained and Bachelor
level social workers are required to deliver required
counseling and case management services for the people
served and their formal and informal support system. Two
Master level trained social workers will be employed to
provide co-occurring counseling services and wraparound

centered around participants and their
families while maintaining collaboration
on team goals. Adding this system will
maximize organizational and natural
support for participants of the POWER
Program while maintaining cultural
competency and an outcome based
foundation.
A qualified consultant for the
Wraparound System will integrate with
the efforts of ABC AGENCY, and its
partners who share case management
responsibilities, to provide training and
guidance. Additionally, because of the
complex nature of patients, a
combination of four social workers are
required to deliver counseling and case
management services.
Two social workers with masters
degrees will be employed to provide co-
occurring counseling services and
Wraparound case management services
to those who qualify and are willing to
receive counseling. These individuals
will be provided counseling and case
management support in accordance with
the State of Florida Outpatient
Substance Abuse Counseling guidelines,
assessed Mental Health Counseling
requirements, and Wraparound case
management guidelines.
Two social workers with bachelors
degrees will be employed to provide
services and support for individuals who
do not qualify for or wish to receive
counseling. These participants will

wishes to engage in both counseling and wraparound
services. These individuals will be provided counseling and
case management supports in accordance with the State of
Florida Outpatient Substance Abuse Counseling guidelines,

Wraparound case management guidelines. Two Bachelor
level wraparound staff will be employed to provide
Wraparound services and supports for individuals who do not
wish to receive or are not assessed to need counseling
services. All participants will receive services in accordance
with applicable Florida State and Council On Accreditation
standards.

Use of the Wraparound System will be
an innovative approach to leading
POWER participants and their support
systems to recovery. ABC AGENCY needs
to expand its model for case
management services to ensure
participants are fully engaged and
supported during their recovery journey.
This will result in not only sobriety but
also provide the best possible quality of
life for participants. The end result will
be the participants integration into the
Recovery Community which will
transition them into life without support
from ABC AGENCY.
