CERTIFICATION

This to certify that GLENN A. PAGADUAN who had been our trainee
hemodialysis nurse since February 23, 2016 to January 16, 2017 as
dedicated and efficient trainee hemodialysis nurse.

Our center practices and observes the same dialysis schedule a hospital
typically has. We have four shifts starting from 2 am to 10pm, Monday to
Saturday. Our hemodialysis nurses usually have 12-hours duty depending on
the numbers of new nurses undergoing an on-the-job training (OJT).

Below are the job descriptions of our Trainee Hemodialysis Nurses:s

Assessment of patients prior to initiation of hemodialysis is important
as this may cause complication and would further harm the patient.
The nurse should then have a good clinical eye.:
A good nursing diagnosis is required where the independent nursing
plan and interventions are based.
Hence, a trainee hemodialysis nurse should know how to make a
nursing diagnosis base on the assessment.
Plan of care must be consistent with the diagnosis and assessment.
The plan must be specific, measurable, attainable, realistic and time-
bound.

Interventions are specific to each patient. Not all hemodialysis patients

have the same cause of CKD. Therefore, a hemodialysis nurse must be
well adept of the nature of illness every patient has.

This certification is issued upon request for general purpose.
