I, WENDEL MARR B. BALIGA, Filipino, of legal age, single and

resident of Balzain Highway, Centro 11, Tuguegarao City, Cagayan, after
having been duly sworn to in accordance with law, do hereby deposes
and states THAT:

1. I am the owner/proprietor of a business establishment (computer

shop) with trade name EDION NET CAFE located at Balzain
Highway, Centro 11, Tuguegarao City;

2. In the conduct of my business, I personally manage the day to

day affairs of my business and no person is currently employed
under me who work in said business establishment;

3. I have executed this affidavit to attest to the veracity and

truthfulness of the foregoing statements and to facilitate securing
of my Phil Health and PAG-IBIG clearances.
