RECREATION THERAPY SPECIFIC PROGRAM PLAN

Program Title: Sports Group Program

Statement of Purpose: Sports group is a program developed to support mental health


service consumers independently take part in physical and social engagement with peers
through sport.

Entrance Criteria: Individuals must be a registered client through Connections


Dartmouth or externally referred for specific program. Individuals must be able to
activity engage in physical activity.

Exit Criteria: Individuals no longer identify as a Connections Dartmouth consumer.

Exclusion Criteria: Individuals, who are uninterested in the program, are unable to
maintain focus for 30-40 min time period or display disruptive behaviors, have issues
mobility.

Terminal Program Objectives:

TPO1. To identify the benefits of physical activity

TPO2. To demonstrate flow through social/physical engagement

TPO3. To provide an environment for social interaction

Enabling Objectives:

EO 1.1 To identify the role of sports for physical health.


EO 1.2 To identify the role of sports for mental health.

EO 2.1 To demonstrate intrinsic motivation


EO2.2 To demonstrate focus on the task at hand

EO 3.1 To demonstrate the ability to take part in team activities


EO 3.2 To demonstrate the ability to build friendships

Performance Measure:

PM1.1.1 Upon completion of a Sports Group session, the individual will identify the role
on their physical health as evidenced by:
A: Indentifying physical wellness prior to participation
B: Identifying physical wellness post participation

PM1.1.2 Upon completion of the Relaxation Group program the individual will identify
the role relaxation plays on their mental health as evidenced by:

a. Identifying mental wellness prior to participation


b. Identifying mental wellness post participation

PM2.2.1 Upon completion of the Sports Group program, the individual will have
demonstrated intrinsic motivation as evidenced by demonstrating:
A: self determination
B: loss of self-consciousness

PM2.2.2 Upon completion of the Sports Group program, the individual will have
demonstrated the ability to focus on the task at hand as evidenced by:
A: participating in an activity without quitting
B: not being distracted by outside influences

PM3.3.1 Upon completion of the Sports Group program, the individual will have taken
part in at least one group/team sports activity

PM3.3.2 Upon completion of the Sports Group program, the individual will have
demonstrated relationship building skills as evidenced by:
A: imitating conversations with peers and or staff
B: independently organizing activities with peers
C: planning sport activities with peers for future programs
