! I think this looks great, and is very complete. Each time

I thought of something that you could add, I scrolled
down and found it. I think this would save us paper and
time, and we would not have to fill out a log entry on
power school along with the referral. - LL
! I would use this form for discipline referral. I appreciate
having the drop down menus. The table for interventions
is a beneficial element. SC

! I would use this form or something like it as a
replacement to the older Office Referral form we have
been using. I like that this form is electronic, it has cells
for grade and attendance (which should bear on
disciplinary discussion), and it has room for photographic
evidence. - RA
! I agree that a uniform discipline program is highly
needed. I like the form because we can print/keep a copy
too. I like that you include grade and missing work and
think that that could provide insight on some behavior
issues, however a quick look on power school and anyone
dealing with the referral can check this and attendance. I
like the drop boxes, I would use the form. - CL
