AFFIDAVIT OF CLOSURE / NON OPERATION

I, ROGELIO P. LUY, of legal age, married, Filipino and a resident

of Block 6, Lot 10, Apitong Street, Virginia Homes, Iponan, Cagayan de
Oro City, after having been duly sworn to in accordance with law,
hereby depose and say:

1. That I am the registered owner/proprietor of a business engaged

in the distributorship of Universal Robina Corporation products;
2. That the business address was at Block 6, Lot 10, Apitong Street,
Virginia Homes, Iponan, Cagayan de Oro City;
3. That the business operations started on February 2002;
4. That sometime in 2003, the above-mentioned business has
ceased to operate due to financial constraints, inadequate income to
sustain the daily operations, and other pertinent matters beyond the
control of the business;
5. That in ROGELIO P. LUY consideration of the
foregoing matters, I Affiant have decided to
discontinue the business operation to
avert from further losses, costs, expenses and charges;
6. That I hereby surrender the business registration certificate of the
said business issued to me by the local government of Cagayan de Oro
City;
7. That unfortunately the aforesaid business registration certificate,
while in my possession, was lost and could no longer be found in spite
of diligent efforts being exerted in order to find it;
8. That I consider the aforesaid business registration certificate lost
and beyond recovery;
9. That I am executing this affidavit to attest as to the truth of the
foregoing facts and for whatever legal purpose/s this may serve.
