TERMS AND CONDITIONS

UCPB Debit Card

By availing of the UCPB DEBIT CARD, you agree to the following terms and conditions, which are in addition to the
provisions covering the UCPB ATM/ ATM Card contained in the Terms and Conditions of UCPB Deposit Products and
Services and Client Information Sheet and other related contracts which you signed upon opening of deposit account/s.

The terms "UCPB", "Bank", "we", "us", or "our" in these Terms and Conditions, shall refer to United Coconut Planters
Bank, a universal banking corporation organized and existing under Philippine laws; while the terms "client",
cardholder, "you", "your", "yours" or "yourself" shall refer to a natural or juridical person or persons availing of the
UCPB Debit Card, and may be interchangeably used to refer to the juridical person's authorized representative.

1. UCPB DEBIT CARD - The UCPB Debit Card (also known as UCPB ATM Card) is a payment card which provides you
with electronic access to your UCPB deposit account/s to withdraw cash and pay for goods and services through
available electronic payment devices. The amount withdrawn or paid shall be deducted from the available
balance of your deposit account/s.

2. CARD ISSUANCE - You may be issued, at the sole discretion of the Bank, the UCPB Debit Card which shall be
used to access your Account/s through any UCPB ATM or UCPBs affiliated payment network/s such as but not
limited to Visa and BancNet. You shall be solely responsible for the security of the UCPB Debit Card issued to
you. You shall claim the Card within THIRTY (30) calendar days from date of application; otherwise, the Card
shall be cancelled automatically by UCPB. You shall shoulder the cost of the replacement card.

3. CARD USAGE - The UCPB Debit Card may be used locally or outside of the Philippines to: (a) withdraw cash
through any UCPB ATM or any affiliated ATM payment network of UCPB, (b) purchase goods and services from
accredited Merchants through Point-of-Sale (POS) terminals or internet payment facilities of any affiliated
payment network of UCPB, and (c) perform the following transactions through UCPB ATMs and affiliated ATM
payment networks of UCPB: (i) inquire account balances, (ii) pay bills, (iii) transfer funds to Bank accounts, (iv)
transfer funds to other local Bank accounts, (iv) change the Personal Identification Number (PIN), and (d) such
other uses as will be allowed by UCPB. The UCPB Debit Card shall be available for use at any time up to the
available balance on your deposit account/s subject to withdrawal limit.

4. TRANSACTION AUTHORIZATION - Your use of the UCPB Debit Card with the required authorization such as
correct PIN entry for ATM and POS terminals, signing on the transaction slip for selected POS terminals or Card
Number and other required information, such as the Card Expiry and Card Verification Value (CVV) entry, for
internet payment facilities/ e-Commerce sites shall serve as your instruction to UCPB to process the transaction
based on your valid authorization.

5. WITHDRAWAL LIMIT - UCPB shall impose a limit to the amount that can be withdrawn from the UCPB Debit Card
regardless of sufficiency of balance of the Account. You agree that while the Incoming Clearing Checks (ICC) are
being posted by UCPB, it may earmark the total amount of ICCs against the available balances in the Accounts.
UCPB may withhold availability of deposit balances to the extent of ICCs until all have been duly recorded.

6. INTERNATIONAL/ FOREIGN CURRENCY TRANSACTIONS - Your UCPB Debit Card can be used in countries outside
of the Philippines through the ATMs or merchants of UCPBs affiliated international payment networks, such as
but not limited to Visa International. For international transactions, you authorize UCPB to debit your account
the equivalent amount in Philippine Peso. For foreign currency transactions, the transaction is converted to
Philippine Peso using a conversion rate applied by Visa International on the date it processes the transaction.
Visa International may convert a foreign currency transaction into US Dollars prior to converting it into
Philippine Peso. Applicable fee/s shall apply to each international transaction made with your UCPB Debit Card
and said fee/s shall likewise be debited from your account/s.

7. TRANSACTION FEES - UCPB may charge fee/s on Debit Card transactions/ usages and revise or impose
additional fees related to the service as it deems necessary. You agree to pay the service charges and fees
presently imposed or may in the future be revised or imposed by UCPB.

8. RESPONSIBILITY OF THE CARDHOLDER AND OWNERSHIP OF THE CARD UCPB will provide a system-generated,
six-digit, initial PIN which you will use to access the ATMs and POS terminals to perform cash withdrawals and
other transactions. You should immediately go to the nearest UCPB Branch or UCPB ATM to change the initial
PIN into a six-digit number of your preference.

You shall at all times:


a. Safeguard your UCPB Debit Card as if it were cash;
b. Be solely responsible for the proper use, custody, safekeeping and security of your Card and PIN;
c. Take all the necessary precautions to protect your UCPB Debit Card from loss and prevent the disclosure
of your PIN to any other parties; and
d. Hold UCPB free and harmless against any and all consequences stemming from the unreported loss,
unauthorized/ fraudulent use, and unauthorized disclosure of your PIN.

9. LOST/ STOLEN CARD - In case of a lost or stolen Card, you shall immediately report the loss to the UCPB ATM
Hotline at (02)811-9292 to ensure proper tagging and blocking of transactions. Pursuant to your sole
responsibility for the safeguarding and safekeeping of the UCPB Debit Card issued to you, all transactions
generated by the use of the lost/ stolen UCPB Debit Card shall be deemed conclusively binding on you, and
UCPB will not be held liable for any damage or loss which you may incur.

10. CANCELLED/ TERMINATED CARD - A UCPB Debit Card which has been deemed by UCPB as having been used for
fraudulent purposes or for irregular or anomalous transactions may be cancelled/ terminated by UCPB without
prior notice to you. If you wish to terminate or cancel your UCPB Debit Card, you shall give UCPB a written
notice THIRTY (30) calendar days prior to the date of actual termination/ cancellation.

11. REPLACEMENT OF LOST, STOLEN, DEFECTIVE/ DAMAGED CARD - UCPB shall replace a damaged/ defective
UCPB Debit Card at no cost to you for as long as the Card has not been used for any transaction. If the card has
already been used for a transaction, UCPB shall replace the damaged/ defective card with a new one and
transfer the remaining value to the new card subject to your payment of the service fees/ charges. For lost or
stolen UCPB Debit Card, UCPB shall require you to present a notarized Affidavit of Loss and a valid identification
card.

12. DISCLOSURE - UCPB shall treat all your information private and confidential. You expressly agree that UCPB may
share your information within the organization, its agents, representatives, advisors, consultants or service
providers for the UCPB Debit Card facility, who shall likewise be obliged to maintain the confidentiality of your
information, and / or to third party organizations in instances where the law or public interest requires it, such
as, but not limited to, the investigation or prevention of crimes or fraudulent transactions, and that UCPB shall
be held free and harmless from any liability that may arise from such disclosure.

You agree to indemnify UCPB against any loss, damage and any such expenses (including, but not limited to,
legal fees) which may be incurred by UCPB as a result of the aforementioned reasons. You shall immediately
affix your signature in the UCPB Debit Card in order to identify yourself as its rightful owner.
