AFFIDAVIT OF EMPLOYMENT

I, ANGELICO ABONITA RAULE, of legal age, Filipino, single, a

resident of Purok 3 Gabon, Polangui, Albay, after having been duly
sworn to in accordance with law, do hereby depose and states that:

1. I am a Registered Nurse by profession;

2. That I am presently employed as Staff Nurse at Dr. Manuel R.

Rebueno Memorial Hospital at Iraya Sur, Oas, Albay, from July
28, 2013 as shown in the Certificate of Employment dated
August 23, 2016 issued by the said establishment, a copy of
which is attached as Annex A;

3. I am executing this affidavit in connection with my application

for employment abroad; and

4. I attest to the truthfulness of this affidavit.
