Larry,

Further to our discussion today, and as per your request, we are forwarding a plan and a photo of
typical cablenets. The photo shows one cablenet installed and another in the process of being
installed.

Our preliminary investigation is pointing towards a failure of the rigging during the installation of one
of the cablenets. There was some resultant damage to the components of the cablenet (cables and
castings) and also to the bleacher area below.

The incident is still under investigation and we expect to have more information in the next few days.

Further to our conversation of just now, I will advise our claims department and Larry of the new
claims. It sounds to me like the majority of the damage is to the new COC materials with some minor
damage to the existing structure. We will report this into the insurers. In the meantime, please

Hi Monica i pass along this notification at PCL's suggestion. Pavco has very little information on the
event at the current time. Here is what we know at this time. During the erection process late friday
afternoon a cable net fell from a crane connection. I dont believe there was any personal injuries
There was some minor damage to existing stadium. I understand from PCL's attached email that the
cable net itself was damaged and some components will require replacement.

I will discuss claim notification protocol with PCL. I suggest JLT's claims department contact Ralph
Herten at PCL for details on this claim.

We have requested Freyssinet to outline their plan for inspection of the elements on cable net R31 that was involved in
the incident last Friday.
Please review and provide any comments or concerns you may have.

We are currently focusing on the installation of R14 and are trying to get confirmation to install R15 after this. This will
hopefully buy us the necessary time to deal with R31.
The next cable net that needs to be installed after R14 and R15 will be R31. We cannot skip this one and continue with
R32 since we will not be able to make the pin connections at the node side at R31 at a later date.
Pins at the node can only be made in a pre-determined sequence because of space restrictions.

First priority would therefore be to get confirmation and approval to use elements from R06 and R24 to re-built cable
net R31.
As noted by Freyssinet the only elements that currently cannot be substituted are the A and B clamps.
Pending your comments on this proposal Freyssinet has therefore started inspection of these clamps.

Don't hesitate to ask for any information that you require in addition.

Quality levels of acceptance are accodlng to the following sketch:


h Red: level 1 (can not be controlled as the plates are now welded)

White (rest of the surface): level 3

understanding is that Christoph was contemplating a site visit this week in any event. Geiger did not ask him to

We have reviewed the inspection program for the R31 cablenet components. We have not yet had the
opportunity to review this with SBP. We may have specific comments pending our review with SBP.

That said, in our view the checklist is and inspection plan is good as far as it goes. We see the key being the
review of the cables and the assessment of those which have been damaged as to whether can be employed in
the work or are to be replaced. We assume Geobrugg's representative will be making an assessment for
Structal/ Freyssinet. Note there is no reason to involve us should you and your subs decide to replace the
damaged cables with new assemblies. If the affected cables are proposed to employed in the Work, we note the
following:

1. We will want to see a written assessment from Geobrugg indicating any repairs required and an unqualified
certification that the repaired cable assemblies meet the specified requirements.

2. We will perform our own review and assessment, Please note this is not a service covered in the scope of
our agreement with PavCo and we will request additional fees for the time and expenses involved.

3. We reserve the right to make the final determination of the acceptability of use of any damaged (repaired or
otherwise) components in the structure. Our standard is that the (repaired) cables employed in the Work
perform as specified, essentially as new.

Anticipate the same approach to the castings and hardware. However, we note that the testing proposed should
be reasonably definitive. Key is the absence of cracks or permanent deformation, We suggest that all clamp
bolts of the castings subjected to impact should be replaced. Similarly the D clamp link pins should be replaced
where they were subject to impact.

We are currently focusing on the installation of R14 and are trying to get confirmation to install R15 after this. This will
hopefully buy us the necessary time to deal with R31.
The next cable net that needs to be installed after R14 and R15 will be R31. We cannot skip this one and continue with
R32 since we will not be able to make the pin connections at the node side at R31 at a later date.
Pins at the node can only be made in a pre-determined sequence because of space restrictions.

First priority would therefore be to get confirmation and approval to use elements from R06 and R24 to re-built cable
net R31.
As noted by Freyssinet the only elements that currently cannot be substituted are the A and B clamps.
Pending your comments on this proposal Freyssinet has therefore started inspection of these clamps.

Don't hesitate to ask for any information that you require in addition.

We have reviewed the attached report, "Inspection Report Net 31", prepared by Freyssinet
documenting the damage to the cables in the Grid R31 cable net assembly that fell during lifting
operations on 3 December 2010. We find the re-use of and repairs to the cables and endfittings
proposed to be acceptable. We note that the cold galvanizing of the damage to the cables zinc
wire coating will not offer quite the same service as the wires' zinc coating. That said, the areas
requiring coating repairs are quite limited.

on the cables during the incident

This part details the inspections led by Freyssinet

3. Geobrugg's inspection report


This part details the inspection led by Geobrugg.

4. Bander's inspection report


This part shows the result of the MPT performed by Elander on the
sockets

Conclusion:
The inspections performed by Freyssinet, Geobrugg and Elander show that no defects on the cables
and sockets.
Touch-up will be performed as per usual procedure.
The cables are good for use as they are.
On his report, the cable manufacturer confirms his warranty.

1 INTRODUCTION

In order to determine the effects of the falling down of Grid 31 during erection we
did a visual test of the cables and of the fittings. Additionally we measured the
thickness of the remaining Galfan coating at the position where scratches were
found. At the end fittings we did magnetic particle inspection carried out by
Elander Inspection.

4 CABLES VISUAL AND GALVANIZATION TEST

Following some of the indicated scratches or little damages at the surface. On


every cable we marked a defect number.

For example upper cable top, defect 1 (UTD1)

Thickness still 60 micro meter. Nominal


about 80. At some spots the thickness
came down to 30 micros.

places on the cable surface. If they didn't
had a smaller zinc layer we did not count
them.

5 CONCLUSION

Freyssinet International & Cie asked us to check the suspension cables of the
Grid 31. Our conclusions are the following:
- The minor damages found on the cables do not have negative influence
on the cables breaking loads, service loads and lifetime.
- At the indicated locations where the thickness of Galfan was measured
lower as nominal, cold zinc spray shall be applied. The surface will serf-
heal from the cathode/anode effect.
- The fittings were MPT tested and no defects were found.
So the R31 suspension cables and fitting still reach the required mechanical
characteristic (MBL) and the required corrosion protection for the project. As a
consequence, the observed elements have no influence on the warranty on our
products.

They have used the components intended for R8 for Installing the net at R26. This shifted the problem from R26 to R8.

They had a spare lower radial on site that it is about 100mm too short for use at R8. To lengthen and use this cable, they
proposed to fabricate longer than normal threaded connector. Geiger and I approved this plan. They expect they can
have this new component on site in 2 weeks.

In the interim they have inventory to continue with CN fabrication and erection at R26,25, R24. They expect to have all
components soon for dropped radial R31 which would allow them to recommence clockwise installation direction.

We lost about a week due to the R26 problem component. Going forward they are working on checking all lengths to
ensure this doesn't surprise us in the future. This is the issue that concerns me most.

the test results from NTB in your 1-17-11 email and forwarded all data to Saint Gobain. B oth Saint
Gobain and ourselves reviewed the information and have concluded the following to date

We applied some grease to both the SF I and Fabrasorb fabrics and have concluded that
the grease will most likely permanently stain the both the outer and liner fabrics.

o Photo 1 is of the sam pies with the grease applied for a period of 5 days, note you
can see the grease stain wlcking into the porous Fabrasorb fabric.
o Photos 2 and 3 are of the fabr ic after it was cleaned with water.
o Photo 4 is of the unbleached SF I fabric after it was cleaned with the recommended
cleaning soap (Murphy's Oil Soap). The dark stain indicates the fabric is
permanently stained. We are bleaching the fabric under UV exposure to get a better
sense of the darkening resultant of the staining.

INTRODUCTION

The full locked cables supplied by our company tend to release some lubricant.
This lubricant is used to conserve the interior steel wires of the cable in order to
protect them against corrosion.

Since two months, the leaking rate tends to decrease, even if, some random
location are still releasing some lubricant

1 CORRECTIVE ACTION PERFORMED

1.1 INITIAL STATUS

Since the beginning of the leaking issue, the status of each cable and the date
of successive cleaning have been recorded.
Based on these data, each cable have been given a note reflecting their
tendency to leak.
The hypothesis was that if a cable is cleaned many times, it means that it is
leaking the most. It is not perfectly accurate as some radials (especially radials
8, 9 and 10) have been dealed with more often due to first installation of fixed
and retractable membranes to avoid drop hits on the first panels. However, due
to the extensive cleaning and number of data recorded, the note reflect correctly
the tendency.

The note has been ponderated, based on the cleaning/ protection method used.
For instance, the upper cables have an higher note as they had to be wrapped
in absorbent pad which was not the case for the lowers. Equivalent ponderation
have been applied to sockets area and lower points (mast, node).

The highest the note is, the more, the related cable was leaking.
Since the end of July most of the cable stop to leak and / or decreased their
leaking rate. Except at specific and isolated area, they are now quite
homogenous. Then the graph below shall be considered as a initial leaking
status illustration only and not current behaviour:

During the cleaning period, some corrective actions have been implemented in
order to contain the drops or to avoid that the accumulating drops fall off the
cable. These actions are described below.

1.2 NODE / UPPER CABLE

At the node area some trays have been installed below the socket to
collect lubricant.
On the upper top and upper bottom cables some gutters have been
installed to collect lubricant.

Those area are then secured and the retractable membrane is protected from
lubricant drop while folded in garage.

13 UPPER CABLE

All the upper top and upper bottom cable have been equipped with
gutters

Those area are then secured. However some clamps (less than 0,1%) still need
some action. The caulking needs to be reinforced to ensure a 100% sealing.
This is monitored on a daily basis by our team, in order to identify any default.

The first heavy rains that occurred after gutter installation showed the good
behaviour and efficiency of drainage system.

1.4 LOWER CABLE / IET TO CLAMP 14

This area is, so far, the best in term of leaking. Very few spot has been noticed
during these last 50 days. It is noticeable that the summer months have been
quite warm (2113 average) and sunny (which bring the cable outer skin
temperature to 40*C) and didn't impact the overall leaking rate.
Furthermore, during the pouring of the Field of Play concrete slab, no drops
have been noticed on the whole area.

An overall cleaning is currently performed not as a necessity but as a part of a


maintenance program. The target being to keep all the cables dry and clean.
It is likely that these area, less loaded than the other part of the cables, will
remain dry in a short future.

IS LOWER CABLE / MAST TO IET

This area tends to improve significantly, in the last 2 months. Few cleaning
operation has been required even if some random location are still leaking.
Especially the socket area and the first meter of cable. Most of the extra
lubricant is accumulating by gravity at these lower points and then induce more
oozing.
Randomly, some other area may leak at a lower level up to significant level. It is
often where the cable is most loaded (i.e. catwalk) or during the tensioning of
fixed membrane connection point.

An overall cleaning is currently performed by our team before the closing of the
membrane.

We have the option in continuing a cleaning maintenance on those area, in


order to avoid punctual drops on the bleachers underneath.

This solution should be satisfactory based on last observation of average


leaking rate of the cables.

2 FURTHER ACTIONS

2.1 GUTTERS

2.1.1 Lower cable at mast location

As it has been observed that the lower cable continue to leak on the lower cable
connection to the mast, we will installed some gutters.
These gutters will run from the bottom of the sockets to the sealing clamp of
fixed membrane.
This gutters will avoid the lubricant to fall in the main drainage gutter.

They will be installed after completion of membrane trade activity, in order to


avoid any damage on them.

A mock-up have been performed to illustrate the shape of this gutter. The
material and principle is identical to initial lower gutter plan.

2.1.2 Cleaning frequency

The cleaning frequency has been recorded since the beginning of the leaking
issue.
In average, all cables have been cleaned 10 times: minimum 5 times, maximum
16 times according to the behaviour of the cable. This extensive cleaning
allowed to remove most of the extra lubricant. However, some area, such as the
most loaded part of cables, the clamps and the lowest point, keep leaking at
lower rate. Some random area are also noticed and are the most difficult to
monitor.

The last months, even though the hottest in term of temperature, the cleaning
was not necessary for most of the cable.

This area is located from the socket fixed to the mast to the first clamp of
membrane edge fixation.
In average 14 days time frame virtually free of drops have been recorded
except at very specific location.
This area remain a significant leaking source. Indeed, being situated at the
lower point, all the extra lubricant within the cable tends to accumulate there.

The area will be equipped with lower gutters at completion of membrane trades
activity to avoid damage on them.
As a temporary measure, the end of the sockets is wrapped in an absorbent
pad.

3 MAINTENANCE

3.1 CABLE PICTURE MAP

In order to illustrate the current status of lower cables during this month "free of
cleaning" a picture map has been made as follow:

All lower cable length from mast to IET pictures taken from the top
All lower cable length from IET to clamp 5 pictures taken from the bottom

It appeared to be not relevant to take picture from the bottom between mast to
clamp 5. Indeed the quality of pictures taken from the bleachers (extensive
zoom) and face to high fight exposure, is very poor. Furthermore, the status of
this area is totally similar to the other ones.

This map is located in appendix.

3.2 MAINTENANCE PROGRAM

Starting September 13th, 2011 up to September 27th, 2011, a daily inspection


will be conducted by a Geobrugg Inspector, in order to optimize the final
intensive cleaning before the completion of membrane installation.
The interest of these daily inspection is:
Identify accurately the potential leaking area to make easier the
scheduled maintenance program
Optimize the remaining period where access to lowers is easier

In a second step, we schedule an inspection and maintenance program for the


following months until the leaking issue definitely cease.

3.2.1 Lower cables from clamo 1 to node

Cycle I:
Inspection once every two weeks
* If no leaking issue noticed, schedule a new inspection 2 weeks
after
- If leaking issue is noticed, plan immediate cleaning and schedule
a new inspection 1 week after

The passage from cycle I to II will be based on 2 inspections without leaking


noticed.

3.2.3 Upper Gutters removal


At this stage, it is not possible to determine accurately the most relevant period
to remove the gutters.
However, a removal of gutters after a period of 2 years can be used as a
reference (end of the cycle Hi)
After removal of gutters a final extensive cleaning will be performed on the
concerned area to remove lubricant stains.

One month after completion of upper gutter removal a final inspection of upper
cables will be conducted to confirm that the cables remain dry.

4 INSPECTION AND CLEANING CREW

The inspection will be conducted by one person.


His role will be to fill up the inspection record sheet
On these record sheets will be indicated the leaking area. A short report will be
added to the record form to define the severity of the leaking, if any, and the
average behaviour of cables.

The cleaning team will be composed of rope access workers able to work on
the cables:
Access to lower cable from mast to clamp 1: from top facade beam with
fall arrest protection
Access to lower cables from clamp 1 to clamp 7: rope access while
walking on the liner from the catwalk
Access to lower cable from clamp 7 to 10: rope access from underneath
the cable
Access to lower cable from clamp 10 to node: rope access, walking on
the cable
Access to upper cables: from the node and then fall arrest protection

The amount of crew will be adapted according to volume of work to perform.


The cleaning intervention will be done on day shift basis during the standard
week working days.
The number of workers mobilized shall be sufficient to complete the cleaning on
maximum 3 days time.
The scheduled cleaning days shall be in accordance with Owner program.

4.1 CONTACT PERSON

A contact person is designated by Geobrugg to manage the coordination of the


maintenance and cleaning operation. He can be contacted by the Owner and
will organize necessary actions:

CLEANING RECOMMENDATIONS

SEFAR* Architecture TENARA* Fabric is 100% fluoropolymer and is naturally dirt-repellent. However,
additional cleaning procedures can be employed to remove stubborn dirt or stains.

Overall Cleaning
In general, most contaminants such as dust and dirt can be deaned with water. If water proves insufficient to
clean the fabric, an ammonia or detergent solution can be used, along with a soft brush and a gentle cleaning
action. It should not be necessary to aggressively scrub" the fabric to remove dirt/ stains. If desired, chlorine
bleach solutions can also be used in any concentration without affecting the fabric. ..Pressure washers" should
not be used, as their extremely high pressure could damage the fabric.

Spot Cleaning
First wipe the discoloration off with a clean damp rag by rubbing gently. If this fails to remove the discoloration,
apply a liberal amount of cleaning solution (as recommended below) to a clean rag and rub the fabric gently.
Use a final water rinse, and allow the fabric to air dry. If desired, dry fabric with a clean rag. This should
remove most dirt spots.
Note: Ammonia and detergents are recommended cleaning agents to try BEFORE isopropyl alcohol, as alcohol
will tend to dissolve the contaminant and could cause it to be absorbed into the top fabric layer.

Cleaning Agents to Avoid


Solvents are NOT recommended and should not be used, as they may adversely affect the stain by helping
to set it" into the fabric. Examples include: Ketones, acetone and methylene chloride, paint removers, oven
cleaners, or dry cleaning solutions. Abrasive cleaning agents or implements should NOT be used as they could
damage the fabric surface.

Common Stains" and Recommended Cleaning Agents


- Amino Acids/Natural fats: Ammonia based products, detergents.
- Common dirt, grease, ink, etc.: Detergents, Isopropyl alcohol.
- Bleach solutions do not affect SEFAR* Architecture TENARA* Fabrics and can be safely used without affecting
the performance of the fabric.
Note: Cleaning agents should be used with care, and if possible tested on a hidden section for adverse
affects. Of course appropriate personal protective equipment should be used as recommended by the
cleaning manufacturer.
