MAPEH ACCOMPLISHMENT REPORT

I. INTRODUCTION

The following are reports on the school activities that took place for the month of
December 2016.

II. STUDENTS DEVELOPMENT

1. Emphasized wearing of uniforms and I.D.

2. Actively participated in extra-curricular activities
3. Actively joined the programs of the school
4. Happily participated, contributed and maintained in the cleanliness of the school.

III. TEACHER DEVELOPMENT

1. Prepared Daily Lesson Logs

2. Involved in different school activities & programs
3. Involved in different community service.
4. Improved and maintained cleanliness of the classroom and surroundings.
5. Religiously conferred with the parents about students development.
6. Acknowledged good and bad comments/criticism.
7. Participated in the Divisions Athletic events.
8. Attended meetings in the Division Office as PESS Coordinator
9. Excellently performed duties as the MAPEH Coordinator

It is indeed a very joyful, colorful and memorable event for the RNHS that will surely be
remembered as the years will come to pass. A heartfelt thank of gratitude to everyone who
contributed to this momentous occasion.
