He was using his oxygen and inhalers as directed and could breathe easier when he wasnt exerting himself. For the medication reconciliation, he showed me the medication list he received from his health care provider and then presented me with his box of medications. Something was missing. His box did not include a medication from his list, which he should have been taking since being discharged from the hospital. It was an anti-inflammatory specifically for helping to prevent COPD symptoms from getting worse. With his permission, I called his primary care provider and confirmed his medication list and explained the missing medication. The nurse taking the call said she would share the information with the members doctor and the office would be back in touch with the member with further instructions. In a follow-up call with the members wife a few days later, I was told his missing medication was filled at the pharmacy and he was taking it as prescribed. Thank you for the visit.
