PETITION

The undersigned petitioner, unto this Honorable Court, most

respectfully states and avers that:

1. He is of legal age having been born on 10 June 1988, single,

Filipino, with Tax Identification No. 276-185-155, and with office and
postal address at the 31st Floor, Litigation Department, LAND BANK
Plaza, 1598 M.H. del Pilar corner Dr. J. Quintos Sts., Malate, Manila 1004;

2. He is a member of the Philippine Bar, having been admitted

as such on 24 April 20151 with Roll of Attorneys No. 64218;

3. He has paid the Professional Tax for Lawyers for the

calendar year 2017 as evidenced by Professional Tax Receipt2 bearing
PTR No. _________________________ dated _________________________ issued by the
Manila Treasurers Office. He has likewise paid the IBP membership
dues for the year 2017 as evidenced by IBPs Official Receipt 3 bearing
O.R. No. 1049299 dated 10 January 2017;

4. He possess all the qualifications and none of the

disqualifications for the appointment to the Office of a Notary Public;

 5. He has never been convicted of any crime involving moral
turpitude and no criminal case against him is pending in the Office of the
City Prosecutor and Regional Trial Court of Manila or elsewhere 4;

6. He is a member in good standing of the Philippine Bar and

has no administrative or disbarment case filed against him as evidenced
by the Certifications issued by the Office of the Bar Confidant 5, IBP
National Office6 and two (2) executive officers of IBP Manila IV Chapter 7;

7. He has no existing commission as Notary Public in any

jurisdiction in the country;

8. Attached as integral part of this petition is a sheet of paper 8

containing three (3) true impressions of his Dry Seal and three
(3)passport size photographs with light background taken within thirty
(30) days prior to application, with petitioners specimen signatures at
the bottom part thereof;

9. In compliance with A.M. No. 02-8-13-SC issued by the

Supreme Court on 6 July 2004 which took effect on 1 August 2004,
petitioner hereby states that his regular place of work as Notary Public
is at the Litigation Department, Land Bank of the Philippines, 31 st Floor,
LAND BANK Plaza, 1598 M.H. del Pilar corner Dr. J. Quintos Sts., Malate,
Manila 10049;

10. He undertakes to inform the Honorable Executive Judge

within ten (10) days of any change in the aforementioned address.

PRAYER

WHEREFORE, it is most respectfully prayed of this

Honorable Court that an Order be issued granting petitioners
appointment as NOTARY PUBLIC for and in the City of Manila,
Philippines.
