At ICICI Wealth Management Inc., we value our relationship with


clients, and strive to understand and serve them better. Your choice of
investment products is important and to help you make the
right choice, we need to know more about you and your approach to
investing.

We have devised a short questionnaire to gauge your investment


attitudes and risk preferences. The questions are aimed at assisting
solely in your discussions with your Relationship Manager on your
investments. Your total score indicates which of the investor profiles
matches your approach most closely.

Your Risk Profile is intended to form the basis of the interaction that
your Relationship Manager will have with you. Your Relationship
Manager will work closely with you to help you structure a suitable
investment strategy for yourself.

We request a few minutes of your time for answering the enclosed


questionnaire.
