Nature of event: Unit withdrawn due to BTL. (Zone - Reheater)

4. Name of First Up, Main Protections & protection on which GCB tripped:
First up : Boiler tripped
Main Protection: Boiler trip, Turbine trip,
GCB Opened on : Low fonruard power relay operated. (86 GT)
S R1 OUservations :- Set was running with 170 MW load. Set withdrawn to attend rear Reheater tube leakage. Main
failure observed at Rear Reheater coil no 1, Tube no 1. Secondary damage observed at Front Reheater coil no 1
Tube no T9,T10,T11 LHS water wall near LRSB 66 LHS T-1RHS tube no T1 & 81
S A1 nemeaiat Action/work done :- All damaged tubes checked and replaced with new spool pieces. Radiography
of all weld joints carried out. Transient zone cleaning done by water jetting. Hydraulic test carried out. Total joint
involved 21 Nos.

6, Root Cause Analysis :- Main failure at rear Reheater coil no 1 ,Tube no 1 due to flue gas erosion.

TJreventi\re action suggested (Short Term) :- Physical survey of accessible area of rear Reheater coils and front
Reheater coils carried out.

8. Preventir/e action suggested (Long Term) :- There was no BTL in Reheater during last 3 years . This is newly
developing pattern of erosion prone area and hence from next overhaul, detail inspection and thickness
measurement of front and rear Reheater coils will be carried out. lnside and outside scale measurement will also be
carried out of Reheater coils and accordingly action will be initiated for replacement / repair work in Reheater zone.
