HAZARDOUS ENERGY TYPE HAZARD


1 ELECTRICAL Shock, Pinch, & laceration

Follow all steps below to perform lockout before servicing machine. Energy LOCATION
1) Notify affected employees before locking out equipment.

2) Follow Standard Shutdown Procedures to deactivate equipment prior


to denergizing and locking out energy isolating device.
3) Pull plug from outlet or Pull main disconnect to off position and lock Electrical 480
with personal lock. Secure key. 1 volts

4) After shutoff - ensure all residual/stored energy, including hot


machine parts, is released and that no unintended motion will occur
before starting work on machine or equipment.

5) If the system cannot be locked out or it fails the verification of energy


isolation, contact your supervisor immediately STOP
6) Perform work. After work is complete, remove tools and unnecessary
equipment. Ensure all guards are in place. Make sure the equipment is
ready to be operated.
7) Reverse above steps #1 - 4 to unlock machine/equipment. Notify all
affected employees. Restart using normal start up procedures.

**Important Reminders**
**Lock out procedures must be followed anytime hands or any other part of your body will be in a point of operation, pinch, or nip point
during tool/die/weldpoint/ or other changeovers including repair, servicing, or adjusting. Lock out is your protection from unintended
machine motion or release of energy that could cause bodily injury. If guarding on the machine needs to be removed to perform any
function mentioned above, the guards must be put back into position prior to running the machine. If you have ANY QUESTIONS
about the lock out procedures STOP IMMEDIATELY and see your supervisor for instructions!

**Every employee working on this equipment must apply his/her lock to all appropriate energy sources for their own protection.

**Any auxilliary equipment such as coil feeders or conveyors must also be locked out if accidental start up or movement could cause
