WHEREAS Producer intends to produce a full-length theatrical motion picture (the Motion Picture)
based on a screenplay tentatively entitled ____________________ written by _______________________
[insert name(s) of screenwriter(s)], which is intended for initial theatrical exhibition.

AND WHEREAS Producer wishes to retain the services of Performer in connection with the Motion
Picture, and Performer wishes to provide such services, upon the terms and conditions herein contained.

TERMS AND CONDITIONS OF EMPLOYMENT

1. Employment. Producer hereby employs Performer to render services to the production of the
Motion Picture in the role of ___________________ (the Role), at the Daily Rate set out in the above
Production / Employment Information, and Performer agrees to render all services customarily rendered
by actors in feature-length motion pictures, at such times and places as are designated by Producer and in
full compliance with Producers instructions in all matters.

2. Term. The term of employment hereunder shall begin on or about the Start Date set out above
and continue until the Wrap Date or such later date as is necessary to complete the photography and
recordation of the Role. Producer guarantees that the period of Performers employment hereunder will
be not less than ____________________ days.

3. Notices. All notices which Producer is required or permitted to give to Performer hereunder must
be given in writing and may be made by personal delivery or by mailing the same addressed to
Performer at the address given in the Performers Personal Information on the face of this Agreement.

4. Personal Information. Performer must inform Producers casting office or the assistant director of
the Motion Picture of any changes to the Performers Personal Information listed on the face of this
Agreement.

5. Wardrobe. The parties agree Performer shall supply all items of modern wardrobe reasonably
necessary for the portrayal of the Role, and that Producer shall supply any so-called character or
period costumes that may be required. For all wardrobe items supplied by Performer, Performer shall
receive a reasonable cleaning allowance and reimbursement for any lost, damaged or destroyed items.
