On Saturday 6/11/16, upon returning home, we noticed that the ceiling was actively

leaking water. At first it was a drip and then it was a steady flow.

We called maintenance who showed up late Saturday night (past 1am Sunday)
maintenance showed up and said that they would work on the problem on Monday
morning.

On Monday morning (6/13/16) maintenance came by again to make a hole in the


roof in order to ascertain the origin of the leak. The pipe was replaced. After
replacing the pipe, the ceiling continued to leak. According to maintenance workers
Jay and Leo, there was a crack in the foundation of the tub of the apartment over
ours (ours is 2C, the leak originated from 3C).

In the afternoon at approximately 1:30, Angel, the head of maintenance came to


observe the origin of the leak. He said that though the foundation of the tub
overhead was leaking slightly, the major source of the leak was the corroded pipe.
Upon looking at the hole in the ceiling, there was a black substance covering the
surrounding wood and surrounding area. The substance appeared to be mold.

Parvez Ahmed, the GM of the Co-op came by to meet with the maintenance people.
He told me that there must have been a break in communication between my wife
and I as he was told that he hadnt communicated us or responded to our emails.

When asked about the mold in the ceiling, I was told by Angel that there is mold in a
lot of places and that the ceiling being open for 1 week would resolve the problem.

Pictures were taken of the mold and of the foundation of the overlying tub. A video
was taken of the active leak.

I asked Mr. Ahmed if he would replace the vanity that had been damaged by the
leak. Mr. Ahmed said that the damage seemed to him to exist before this incident.
He said that the building was built in 1947 and therefore suffered from problems
that an older building is prone to.

According to the work receipt, the work was summarized as, breach ceiling in the
bathroom to check a leak. Replaced tub trap of 3C. Applied epoxy to trap water.
Work assigned to Jay and Leo. Work noted as completed.
