AFFIDAVIT OF DISCREPANCY

We ALEXIS M. VILLASIS, Filipino, 62 years old, married, and a resident of Brgy.

Malinao, Can-avid, E. Samar and AMALIA C. VILLASIS, Filipino, 50 years old, married
and a resident of Brgy. Malinao, Can-avid, E. Samar after having been duly sworn to in
accordance with law, hereby depose and say:

1. That we know personally ALMIE CABALLA VILLASIS;

2. That she was born to the spouses ALEXIS M. VILLASIS and AMALIA C.
VILLASIS on September 27, 1987 at Mondragon, Northern Samar,
Philippines;

3. That however, in her LIVEBIRTH, her Place of Birth was erroneously entered
as _______________ instead of Mondragon, Northern Samar, which is
correctly reflected in her Form 137;

4. That we personally know these facts , because we have been long time
friends and acquaintances with the said _________________ and his/her
family;

5. that we are executing this affidavit to attest to the truth of the foregoing facts
and for whatever legal purpose the same may serve.
