AFFIDAVIT

I, JOEL D. TIEMPO, of legal age, Filipino, married and a resident of

Purok Maharlika, Cuambogan, Tagum City, Davao del Norte, Philippines, after
having been duly sworn to in accordance with law, hereby depose and state:

1. That I am working as a Security Guard in Jalee Pharmacy at

Rizal St., Barangay Magugpo Poblacion, Tagum City, Davao del
Norte under the agency of MINVETS Security, Inc., Tagum
Branch, Iga Uy St., Tagum City;

2. That my shifting schedule for work begins at 6:00 PM to 4:00

AM, Mondays to Sundays;

3. That on October 17, 2016 at about 7:40 AM, Stewart Chandler

C. Dizon, an employee of the said pharmacy reported to the
Tagum City Police Station that it was forcibly opened and rob;

4. That during the tour of my duty in that time, I did not notice and
hear any unusual noise inside the said pharmacy;

5. That I am executing this Affidavit to attest to the truth of the

foregoing facts and for whatever legal purpose it may serve.
