AFFIDAVIT OF DISCREPANCY

I, ARACELI TAGHOY RAMIREZ-BANGUIS, of legal age, Filipino, widow and

a resident of Block 9 Lot 32, Capitol Homes, Mankilam, Tagum City, Davao del
Norte, Philippines, after having been duly sworn to in accordance with law, hereby
depose and state:

That I am the same ARACELI TAGHOY RAMIREZ-BANGUIS who was born

on December 4, 1964 at Sto. Tomas, Davao;

That in my Certificate of Live Birth, my first name appeared as ARACELLI

instead of ARACELI, which is my true and correct first name as appearing in my
Certificate of Marriage and other important documents;

That the first names ARACELLI and ARACELI refer to one and the same
person;

That on April 18, 1995, I contracted marriage to RUBEN DECIPOLO

BANGUIS (now deceased) at Our Lady of Fatima Parish, 3rd Avenue, Teachers
Village, Tagum, Davao, solemnized by Rev. Fr. Reynaldo A. Biliran;

That in the Certificate of Live Birth of my late husband and in our Certificate
of Marriage, my husbands middle name appeared as DECEPULO instead of
DECIPOLO, which is his true and correct middle name as appearing in his
Government Service Insurance System (GSIS) records, PhilHealth records and
other important documents;

That the middle names DECEPULO and DECIPOLO refer to one and the
same person;

That I am executing this Affidavit to attest to the truth of the foregoing facts
and in connection with the requirement of Tagum Cooperative, Country Bankers
(Rural Bank of Compostela), Government Service Insurance System (GSIS) and
Home Development Mutual Fund (HDMF)Pag-IBIG Fund for the death claim
benefits of my late husband, and for other legal purposes this may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 29 th day of

September, 2016 at Tagum City, Davao del Norte, Philippines.
