I, AIREEN S. ALVIOLA, Filipino citizen of legal age, single and with residence
at Purok Matinabangon, Barangay Labangal, General Santos City, after being duly
sworn to in accordance with law, hereby depose and say THAT:

1. AMANDO DECIERDO MABANO, JR. is a 1st cousin of my deceased father,

CELESTINO DECIERDO ALVIOLA who died on October 3, 2012;

2. AMANDO DECIERDO MABANO, JR. is residing in Toril, Davao City, Philippines;

and

3. As such, I am executing this Affidavit to attest to the truthfulness of the

foregoing, and for whatever legal purposes it may serve.
