WAC 296-823-12005(2)
The employer did not ensure all employees with occupational exposure to bloodborne pathogens were provided
with training before assigning tasks where occupational exposure might occur, and at least annually within one
year of the previous training.
Cited for not providing initial and annual bloodborne pathogen training for all employees with occupational
exposure at Best Western located at 32124 25th Ave S. Federal Way, WA 98003.
Violation is a failure to abate from the previous inspection since the same employees had not received blood
borne pathogen training are still with the employer. Employees interviewed stated they had not received
training, and there is no evidence that the employer had trained employees.
Without initial and annual refresher training, employees may not know or forget essential information needed to
protect themselves from exposure to Bloodborne Pathogens. This increases their risk of contracting HIV, HBV,
or other disease-causing organisms potentially leading to lifelong illness or death.
The violation above is cited as a Failure to Abate of violation # 1, Item #1, cited on
Citation and Notice #317943105 issued on 12/29/2016.
This violation was corrected during the inspection.

WAC 296-823-13005
The employer did not ensure the Hepatitis B vaccination series was available to all employees who have
occupational exposure to bloodborne pathogens.
Cited for not providing the Hepatitis B vaccination series to employees such as housekeepers, and maintenance
workers who are exposed to blood or other potentially infectious material.
Exposure to infected blood or other potentially infectious material can result in contracting an infectious disease
such as HIV or Hepatitis B or Hepatitis C; these diseases can cause permanent disability and death.

The employer did not ensure emergency washing facilities are readily available and functional.
Cited for not having an emergency eyewash station in the kitchen, pool chemical room and laundry room where
employees are exposed to corrosive, toxic, or strong irritants. This is failure to abate from the previous
inspection, since the employer had not installed any emergency eyewash stations in the kitchen, laundry area, or
the pool area at the facility.
Without an OSHA compliant emergency eye washing station, employees can suffer severe eye tissue damage in
the event of accidental contact with corrosive or irritating chemicals used at the work place.
Citation and Notice #317943105 issued on 12/29/2016. You must provide proof of
correction by the correction date shown below. Failure to meet this date may result in
penalties.

The employer did not developed, implemented, or maintain a written hazard communication program which at
least describes how the criteria specified in WAC 296-901-14012, 296-901-14014, and 296-901-14016, for
labels and other forms of warning, safety data sheets, employee information, and training will be met.
Cited for not having a written hazardous communication program for Best Western at the time of the inspection.
Employees work with hazardous chemicals which include corrosive, and toxic chemicals.
Without an employer established written hazard communication program employees are unlikely to be familiar
with the proper personal protective equipment is needed to safety perform their duties and protocols to follow in
the event of accidental exposure.

WAC 296-800-14005
The employer did not develop a formal, written accident prevention program. You must develop a formal
accident prevention program that is outlined in writing, and is tailored to the needs of your workplace, and to
the types of hazards involved.
Cited for not having written accident prevention program that was tailored for the hotel industry.
Without a developed and tailored accident prevention program, employees may be more likely to be placed in
an unsafe situation, which may result in serious injuries.

The employer did not ensure a safety committee was established with elected members.
Cited for not having a safety committee with elected members.
Employees who do not benefit from an effective safety committee, and relevant safety information discussed
during safety committee meetings, are more likely to be injured due to lack of knowledge of hazards or methods
to prevent or control those hazards.
Citation and Notice #317943105 issued on 12/29/2016. You must establish a safety

- Has employee-elected and employer-selected members.
- The number of employee-elected members must equal or exceed the number of
employer-selected members.
- If there is an employee-elected member vacancy, a new member must be elected

WAC 296-800-16005
The employer did not ensure a hazard assessment for personal protective equipment (PPE) was conducted.
Cited for not identifying hazards or potential hazards related to the hotel/motel industry. Hazards that must be
addressed to assess appropriate personal protective equipment needs for employees to include, but is not limited
to, blood borne pathogens, chemical exposures and emergency washing stations.

WAC 296-800-15020
The employer did not provide appropriate first aid supplies readily available to employees.
Cited for not having fully stocked first aid kits in the kitchen. Employees may be exposed to chemical burns,
cuts, and other low grade accidents that may require first aid supplies.

The employer did not ensure that all electrical fittings, and boxes were maintained in good condition for the
purpose of preventing accidental contact with energized electrical components.
Cited for having two instances where faceplates were missing or broken in the kitchen area. This exposes
employees to accidents involving electrical shock hazards from accidental contact with energized electrical
components.


WAC 296-800-30020
The employer did not ensure that the portable fire extinguishers are visually inspected monthly, and the
employer did not ensure that maintenance checks are conducted annually. In the event of a fire, employees
aren't assured of proper working fire extinguishers if they aren't inspected monthly, and are not maintained
annually.
Cited for not inspecting at least two fire extinguishers in the facility, monthly, and not maintaining it annually.

WAC 296-27-02105(1)(d)
The employer did not post an OSHA 300-A Summary Log or equivalent form at the end of each calendar year.
Cited for not posting the OSHA 300-A Summary for 2016. The employer had at least eight work-related
recordable injuries in 2016.
At the end of the calendar year, the employer must create and post an annual summary from February 1st to
April 30th the following year.
Situation not believed to exist any longer. However, if this violation is
identified again during future inspections, it may result in repeat or failure to
abate violations which may include penalties.

Appeal Rights
For Employers
If you are cited for a violation of Occupational Safety and/or Health rules, you have the right to appeal the citation. You
have 15 working days from the date you receive this citation to appeal. (RCW 49.17.140(1)) "Working day means a
calendar day, except Saturdays, Sundays and all legal state holidays. Your appeal must be in writing. It may be mailed,
faxed, or personally delivered.
For violations classified as serious, willful, repeat serious, or failure to abate serious, an employer must correct the violations by the
