WAC 296-823-12005(2)

The employer did not ensure all employees with occupational exposure to bloodborne pathogens were provided
with training before assigning tasks where occupational exposure might occur, and at least annually within one
year of the previous training.

Cited for not providing initial and annual bloodborne pathogen training for all employees with occupational
exposure at Best Western located at 32124 25th Ave S. Federal Way, WA 98003.
Violation is a failure to abate from the previous inspection since the same employees had not received blood
borne pathogen training are still with the employer. Employees interviewed stated they had not received
training, and there is no evidence that the employer had trained employees.

Without initial and annual refresher training, employees may not know or forget essential information needed to
protect themselves from exposure to Bloodborne Pathogens. This increases their risk of contracting HIV, HBV,
or other disease-causing organisms potentially leading to lifelong illness or death.

Cited for not providing the Hepatitis B vaccination series to employees such as housekeepers, and maintenance
workers who are exposed to blood or other potentially infectious material.

Exposure to infected blood or other potentially infectious material can result in contracting an infectious disease
such as HIV or Hepatitis B or Hepatitis C; these diseases can cause permanent disability and death.

WAC 296-800-15030

The employer did not ensure emergency washing facilities are readily available and functional.

Cited for not having an emergency eyewash station in the kitchen, pool chemical room and laundry room where
employees are exposed to corrosive, toxic, or strong irritants. This is failure to abate from the previous
inspection, since the employer had not installed any emergency eyewash stations in the kitchen, laundry area, or
the pool area at the facility.

Without an OSHA compliant emergency eye washing station, employees can suffer severe eye tissue damage in
the event of accidental contact with corrosive or irritating chemicals used at the work place.
The violation above is cited as a Failure to Abate of violation # 1, Item #5, cited on

least describes how the criteria specified in WAC 296-901-14012, 296-901-14014, and 296-901-14016, for
labels and other forms of warning, safety data sheets, employee information, and training will be met.

Cited for not having a written hazardous communication program for Best Western at the time of the inspection.
Employees work with hazardous chemicals which include corrosive, and toxic chemicals.

Without an employer established written hazard communication program employees are unlikely to be familiar
with the proper personal protective equipment is needed to safety perform their duties and protocols to follow in
the event of accidental exposure.

Without a developed and tailored accident prevention program, employees may be more likely to be placed in
an unsafe situation, which may result in serious injuries.

Employees who do not benefit from an effective safety committee, and relevant safety information discussed

to prevent or control those hazards.

The employer did not ensure that all electrical fittings, and boxes were maintained in good condition for the
purpose of preventing accidental contact with energized electrical components.

Cited for having two instances where faceplates were missing or broken in the kitchen area. This exposes
employees to accidents involving electrical shock hazards from accidental contact with energized electrical
components.

aren't assured of proper working fire extinguishers if they aren't inspected monthly, and are not maintained
annually.

WAC 296-27-02105(1)(d)

The employer did not post an OSHA 300-A Summary Log or equivalent form at the end of each calendar year.

Cited for not posting the OSHA 300-A Summary for 2016. The employer had at least eight work-related
recordable injuries in 2016.

At the end of the calendar year, the employer must create and post an annual summary from February 1st to
April 30th the following year.
Situation not believed to exist any longer. However, if this violation is
identified again during future inspections, it may result in repeat or failure to
abate violations which may include penalties.
