ABSTRACT

BackgroundDiabetes mellitus prevalence has been


increasing all over the world as in Indonesia. This
chronic disease needs a continous and comprehensive
management including medical and non medical
intervension. In Indonesia since 2010 PT. Askes as the
national health insurance company has developed Program
Pengelolaan Penyakit Kronis (Prolanis), a program to
improve quality of type 2 dibetes mellitus management
by general practitioners by providing training and set
a monitoring system for diabetes treatment. General
practitioners as the first gate of primary care should
be able to perform an effective management of type 2
diabetes mellitus
Objective The aim of this study is to know the
effectiveness of the implementation of Program
Pengelolaan Penyakit Kronis (Prolanis) by general
practitionersin managing type 2 diabetes mellitus in
Turi District, Sleman, Yogyakarta.
Method Data from medical records and diabetes
monitoring book of 52 patients in a private general
practitioner clinic for 24 months were taken and
analyzed. There was also a survey conducted to explore
the patients lifestyle and education received by them
from their physician.
ResultThere is no significant decreased of the trend of
fasting blood glucose from 2012 to 2013. The trend of
HbA1c level increase from 2012 to 2013. At the end of
2012, the percentage of diabetic patients reaching
target levels of fasting blood glucose were 24% and
increased at the end of 2013 as much as 38%, but the
percentage of patients reaching target levels of HbA1c
levels decreased from 35% to 26%.
ConclusionThe implementation of Prolanis in managing
type 2 diabetes mellitus by general practitioner is
less effective based on the trend of fasting blood
glucose, increasing trend of HbA1c level, and the
percentage of diabetic patients reaching target levels.
