Heart Failure (HF)

Heart failure patients with documentation in the hospital record that left

hospitalization, or is planned for after discharge

Hospital-Based Inpatient Psychiatric Service (HBIPS)

Psychiatric patients who were placed in physical restraints during their inpatient
hospitalization. This measure will determine the total number of hours that

based inpatient psychiatric setting

Psychiatric patients who were placed in seclusion during their inpatient


hospitalization. This measure will determine the total number of hours that all

patients were maintained in seclusion for those admitted to a hospital-based
inpatient psychiatric setting.

Nursing-Sensitive Care (NSC)

Patients that have hospital-acquired (nosocomial) pressure ulcer(s) (category/stage


I-NSC-2 II) on the day of the prevalence study.

All documented falls with or without injury, experienced by patients in a calendar
