STEPS ON HOW TO REQUEST

TRANSCRIPT OF RECORD (Form 137)

I. For Transferees and Graduates

1. Present your Request Forms from the present school
you are connected with.
2. Show any valid I.D for release.

II. Employment
A. For LOCAL Employment
1. Present list of requirements for employment
2. Show valid I.D for release

B. For Employment ABROAD

1. Present the list of requirements for employment
abroad.
2. Show your original Diploma.
3. Wait for the computerized T.O.R and some other
important documents.
4. Provide a photocopy (3 copies each) of the important
documents such as Certification, List of Graduates,
Diploma and TOR.
5. Return all the photocopied documents to the
respective personnel for the school seal and
principals signature to the certified true Xerox copy.

REQUIREMENTS FOR CORRECTION OF NAME /

BIRTH DATE / BIRTH PLACE
(Original and two (2) photocopies of the
following)
1. Letter of request for the correction of NAME / BIRTH
DATE / BIRTH PLACE / etc. Duly signed by the person
making the request and notarized by a notary public or
person authorized to administer oath.
2. Endorsement letter (request) from the school for
correction of NAME / BIRTH DATE / BIRTH PLACE / etc.
or certification from the school as to date of
graduation.
3. Original Birth Certificate issued by the Local Civil
Registrar of place of birth or Office of the National
Statistics Office.
4. Joint Affidavit of two (2) disinterested persons.
5. Original or Certified True Copy of Form 137 and
Diploma.

REQUIREMENTS FOR CAV-PUBLIC SCHOOLS

(Certification, Authentication and Verification of
School Records)

1. DIPLOMA Original + 3 xerox

2. Form 137/ Transcript of Tecords Original + 3 xerox
3. Certification of Graduates (for graduates) Original + 3
xerox
4. Certification of Last School Attended (for non-
graduates)
5. List of Graduates Original + 3 xerox
6. Birth Certificate (NSO or Local Civil Registrar Issued)
7. Two (2) pcs. 2x2 pictures (recently taken)
After completion of the requirements proceed to the DepEd
Region IV Office, Pasig City and present at least two (2)
valid I.Ds.
