Dear Sir/Madam,

My name is Teresa V. Castro, 80 years old, a permanent resident of the United States and currently
residing in the Philippines. I am writing this letter to request for the extension of my re-entry permit that
will be expiring on February 2, 2017. I am scheduled to leave the Philippines for the United States last
October 15, 2016 but had to defer my trip due to my health condition.

I am enclosing a copy of my re-entry permit and my medical certificate for your reference.

I trust that you will grant my request to extend the expiration date of my re-entry permit to allow me
