New Application
Full set copy of passports (including empty pages). Please
ensure copies are clear and readable.
Renewal Application
Requirements Identity (ID) and bearers signature page of the passport
Current valid pass (i.e Dependent Pass, Special Pass)
Latest entry stamp
Observation/Amendment page (if any additional remarks
stated by the Authority)

NOTE The validity of the passport must be more than 6 months

upon submission of application to e-Xpats Service Centre.
The passport must have a minimum of 6 consecutive empty
pages.

2. Birth Certificates for children above 18 years old

If in other languages, please translate it into English before

NOTE certification by respective Embassy in Malaysia OR Malaysian
Embassy in the respective country.

FOR CHINA & BANGLADESH NATIONALITY

To submit Original Certified True Copy of birth certificate
(children above 18 years old) by the respective Malaysian
Embassy/High Commission/Consulate General in China/Bangladesh
If in other languages, please get it translated into English by the
authorised agency
FOR IRAN, IRAQ, NORTH KOREA Nationality
To submit original Certified True Copy of birth certificate
(children above 18 years old) by the Malaysian Embassy / High
Commission/Consulate General in respective country OR respective
Embassy in Malaysia.
If in other languages, please get it translated into English by the
authorised agency
 Affidavit issued by authorised agency i.e respective embassy in Malaysia,
applicable for parents/parents in law ONLY

Affidavit refers to Confirmation of relationship.

Custody Letter is applicable for step children/ adopted children ONLY
