END STAGE RENAL DISEASE A CASE PRESENTATION


ON INTRODUCTION As health care providers, it is our duty to know the patients outlook on
the disease process. We are the ones who are actively involved in providing care to patients in a
holistic approach. Aside from our common duties such as assessing the quality and degree of the
patients condition, assessing and logging symptoms and treatments, we as nurses, go deeper. We
also include emotional and spiritual support to patients and their families, as well. Our common
goal is to understand the outcome of the nursing management rendered to patients with kidney
disorders. These are the reasons why this case study was made.
INTRODUCTION continuation... End-stage kidney disease (ESRD) occurs when the kidneys are
no longer able to function at a level needed for day-to-day life. It usually occurs when chronic
kidney disease has worsened to the point at which kidney function is less than 10% of normal.
ESRD almost always follows chronic kidney disease. A person may have gradual worsening of
kidney function for 10 - 20 years or more before progressing to ESRD. ESRD being the top 10
killer in the Philippines as of 2007 and is increasing that its morbidity and mortality rate is
predicted to double by year 2010, according to DOH. As said in the news reports as of November
2008, that health experts raised an alarm over the rising kidney diseases in the country, saying
that more than 10,500 Filipinos are afflicted with it. There are cases of children that account for
10 to 15 percent of the end-stage renal disease (ESRD), primarily because of their lifestyle-diet
that is high in salt or sugar and due to lack of exercise. Theoretical Framework The theory of
Jean Watsons Human caring theory Human caring in nursing is not just an emotion, concern,
attitude, or benevolent desire. Caring connotes a personal response. Caring promotes health more
than does curing, caring can be demonstrated only interpersonally and effective caring promotes
health. Nursing Education

This study can be a useful learning guide in nursing education as this can be used by the students
as a reference for future studies regarding Chronic Glomerulonephritis and related cases. This
case study will enable the students to learn how to assess patients with any signs of this disease
and be able to provide appropriate nursing care and management.
Significance of the study Nursing Practice

This case study can be used as a tool in nursing practice because it provides nursing interventions
for patients with Chronic Glomerulonephritis. This study can give a good intervention to the
disorder so that an established nursing action can be quickly utilized. Through this study,
important information regarding this illness has been gathered this will be helpful on the students
to have an understanding on the said disorder. Nursing Research
This case can be used as a baseline data for further research of the current management of
patients with Chronic Glomerulonephritis. There might be some information in this study that
can be a good use for research studies. It is important to do research to gain new information,
better interventions and techniques to provide to the patients. General Objectives

This case study will enhance the students knowledge regarding the health and disease condition
of a patient with Chronic Glomerulonephritis, its disease process, possible complications,
treatment plan, and medical-nursing interventions.
Objectives of the study The group aims to:
1. A thorough assessment of the client which includes physical assessment and family.
2.Identify signs and symptoms exhibited by a patient with Chronic Glomerulonephritis.
3.Explain and elaborate the nature of the disease process.
4.Provide appropriate and proper nursing diagnosis in line with the clients medical condition
and skillfully formulate nursing care plans for the problems identified.
5.Apply nursing interventions necessary for the patients condition in reference with the learned
theories and concepts of the disease.
