AFFIDAVIT OF DISCREPANCY

I, RAMON S. SANTELICES, of legal age, married, Filipino citizen, and a

resident of 33 Estacio Subdivision, Martirez Del 96, Pateros Metro Manila, after
having duly sworn in accordance with law, hereby depose and state that:

1. I was born to Spouses Leovigilda Sorreda and Aanselmo R. Santelices

on 31 December 1956 at Calolbon, Catanduanes. The fact of my birth
was registered with the Local Civil Registrar of Calolbon, Catanduanes;

2. To my knowledge, my date of birth is on 31 December 1957 and I have

been consistently using the same in all my records and business
transactions.

3. Recently, however, I discovered that in my Certificate of Live Birth, the

true and correct date of my birth is on 31 December 1956 and NOT on
31 December 1957;

4. I am executing this affidavit for the purpose of attesting to the truth of

the above and foregoing statements and for whatever legal purpose it
may serve.

IN TRUTH WHEREOF, I have hereunto set my hand on this 18th day of

August 2017 in the City of Makati.
