INFORMATION FOR CONTRACTORS

The Saudi Arabian Oil Company (Saudi Aramco) hereby solicits prequalification for the work
described below. A copy of contractors Saudi Arabian Government commercial registration
certificate or ministerial license permitting contractor to perform the type of work to be
accomplished under this contract must be on file with the Contracting Department prior to
contract award.

Title: General Bid Slate for In-Plant Construction


Location of Work: Kingdom of Saudi Arabia

Brief Description of Work:

Saudi Aramco plans to develop and/or upgrade a general bid slate for In-Plant Construction
Projects to be performed in Saudi Arabia.

The type of work generally required for In-Plant Construction projects will mainly consist of
construction of In-Plant piping, underground utilities and installation of pumps and
compressors.

Contractor shall furnish all labor, equipment and material required to perform the work.

Prequalification Requirements:

Interested contractors that can demonstrate successful and verifiable contract


experience and capabilities in this type of work are requested to complete the related
Prequalification Questionnaire and submit all required forms and information in softcopy
(on CD, 3 copies) using Adobe Acrobat (Acrobat Writer for text and Acrobat Distiller for
graphics).

The 3 CDs must be contained in a sealed envelope. The 3 CDs and the envelope must be
clearly marked as follows:

To be considered, contractor must have sufficient specialized experience in this type of work
and adequate numbers of qualified manpower and equipment.

A Saudi Aramco Prequalification Evaluation Team will evaluate all submittals fairly and in
accordance with the applicable evaluation criteria. This will include verification of contractor
information and visits to contractor work sites. Contractors that will be qualified by Saudi
Aramco and are deemed suitable for the work will be notified and will be included in the General
Bid Slate. Contractors that do not receive such notification may request a formal debriefing in
accordance with Saudi Aramco Contracting Department standard procedures.

1. GENERAL PRE-QUALIFICATION REQUIREMENTS

1.1 The information requested in this Questionnaire shall be submitted by each


contractor.

1.2 All information requested shall be provided completely and explicitly


responding to each and every item specified under each evaluation category
listed below.

1.3 The attached forms shall be used as instructed. Continuation sheets of the
attached forms may be used to provide the information requested, if necessary.
All information requested in the forms must be provided.

1.4 Responses to each evaluation category must clearly and specifically reference
the item(s) or sub-item(s) for which the response is given. The responding
contractor or sub-contractor name must appear on each page of all required
forms submitted.

1.5 Include a cross reference table with the submittal, which specifically
identifies each item and its corresponding response(s).

FAILURE TO SUBMIT RESPONSES IN STRICT COMPLIANCE WITH ALL


THE REQUIREMENTS SPECIFIED IN THIS QUESTIONNAIRE MAY BE
CAUSE FOR DISQUALIFICATION.

1.7 Responses not received before the mentioned deadline and in the manner
specified above, for any reason, will not be evaluated. Company brochures
and /or catalogs will not be accepted as prequalification submittals. However, it
may be submitted by Contractors as extra information only.

1.8 Possession of a license and/or commercial registration in Saudi Arabia is not a


pre-requisite for a contractor being placed on a Saudi Aramco Bid Slate.
However, Saudi Aramco will award contracts for work being performed in Saudi
Arabia only to those contractors who, by the bid closing date, are properly
licensed and/or commercially registered by the Saudi Arab Government
Authorities and are registered with Saudi Aramco in the Electronic Contracting
Network (ECN).

2. EVALUATION CATEGORIES

Saudi Aramco will evaluate each contractor. This evaluation will assess technical
capabilities to determine suitability for performing this type of work.

Contractors Pre-Qualification Proposals will be primarily evaluated as follows:

Relevant Experience as it relates to In-Plant General Work.


Quality, capability and organization of management and work force.
Previous performance.

The Pre-Qualification Evaluation Categories delineated in Items 2.1 through 2.13 below
are not necessarily listed in order of importance. Under each category are items and
sub-items. These either define the information being requested in the category or list
statements requiring specific responses.

2.1 COMPANY ORGANIZATION/MANAGEMENT (EXHIBIT "A")

2.1.1 As Exhibit "A", provide an Organization Chart (in hierarchical


management format) that as a minimum: (a) identifies the various
functional departments within your company, and (b) provides the names
of personnel currently holding department management through
executive management positions.

2.1.2 The Organization Chart must also clearly identify the specific person (that
is, the Project Manager) who will be responsible for overseeing Saudi
Aramco projects. Show the direct lines of interaction between the Project
Manager and your company's department /executive management
personnel as requested under Paragraph 2.1.1 above.

2.2. AFFILIATED COMPANIES/SUBCONTRACTORS (EXHIBIT "B")

2.2.1 Under Exhibit "B", list the names and addresses of all affiliated firms
and/or potential sub-contractors that will be working with your company
on any In-Plant General Work. You are required to identify both the type
and the approximate percentage of work to be performed by your
affiliated firms and/or sub-contractors. Use a separate sheet for each
affiliated company and/or sub-contractor.

2.2.2 If your company has no affiliated firms and you do not intend to
subcontract any portion of the work, you must provide a clear written
statement of this fact and submit the statement as Exhibit "B".

2.3. KEY PERSONNEL (EXHIBIT "C")

2.3.1 Under Exhibit "C" provide resumes for each of the key positions listed
below:

Site Manager
Quality Control Manager/Discipline QA/QC Supervisors (Electrical,
Civil, Mechanical, Piping, etc)
Safety Manager
Procurement/Materials Handling Manager
Cost/Scheduling Engineer
Civil/Structural Superintendent(s)
Piping/Mechanical Superintendent(s)
Electrical/Instrumentation Superintendent(s)

2.3.2 Resumes are to address educational background, as well as the number


of years of experience in all positions held to date. Limit your response to
no more than two (if applicable) representative candidates for each job
category. Additional resumes, or resumes for positions not
requested by this Prequalification Questionnaire, will not be given
consideration.

Please note that some of the key personnel identified above may be
referred to by several different titles within the business community. In
order to receive credit, be sure that your resumes clearly identify the
candidates by the positions requested under this Prequalification
Questionnaire.

2.4. MANPOWER RESOURCES (EXHIBIT "D")

2.4.1 Complete Exhibit "D", by providing details of your company's current and
projected manpower resources for the next three (3) years.

2.4.2 As Exhibit "D-1", provide the requested information indicating your


company's ability to recruit a workforce should it be necessary.

2.4.3 As Exhibit "D-2", list by name all your company's heavy equipment
operators licensed by the Saudi Arabian Government. In support of this
information, you are required to submit a copy of the license for each
named operator.

Your heavy equipment operators will not be evaluated if you fail to


provide the requested supporting documentation. In turn, this may have
an adverse impact on your company's overall evaluation.

2.5. PURCHASING AND MATERIAL HANDLING (EXHIBIT "E")

2.5.1 Complete Exhibit "E" by providing information on: the company's


employees involved in material procurement and handling; the names
and locations (both in-Kingdom and out-of-Kingdom) of any affiliated
purchasing agents; and lastly, the approximate value of permanently
installed plant, equipment and machinery purchased in-Kingdom each
year for the past ten years.

2.5.2 As Exhibit "E-1" provide a flow chart (developed in your own format)
showing the steps your company follows from the time a quote is
requested, until the equipment or material is delivered at the Work Site.

2.6. COST AND SCHEDULING (EXHIBIT "F")

2.6.1 Complete Exhibit "F" by providing information on: the company's


employees involved in your cost and scheduling unit and state whether
your company utilizes computers in its cost and scheduling functions.

2.6.2 As Exhibit "F-1", provide a copy of a detailed Project Schedule (CPM)


prepared by your company for one of the recent projects listed under
Exhibit I.

2.7. ACCOMMODATIONS (EXHIBIT "G")

2.7.1 On Exhibit "G", list all accommodations (particularly, any camp facilities)
that your company provides for its workers.

2.7.2 Also on Exhibit "G", briefly describe how your company proposes to
provide accommodations when performing projects at various Saudi
Aramco Work Sites. Your description should indicate to Saudi Aramco
that you have taken into consideration such elements as: transportation,
catering, medical coverage, recreation, etc.

2.8. EQUIPMENT (EXHIBIT "H")

2.8.1 Complete Exhibit "H" by listing all the major equipment owned by your
company. Equipment listed under Exhibit "H" must be in good operating
condition and is subject to verification by Saudi Aramco site visits.

If a site visit is conducted and Saudi Aramco determines that the


equipment is either unavailable, or is in a non-operational condition, your
company will be eliminated from further consideration for inclusion on this
Bid Slate.

2.8.2 As Exhibit "H-1", describe your company's equipment maintenance and


storage facilities. As a minimum, your description must include the
locations (maps are helpful) and sizes of your facilities, as well as the
number and occupations of employees assigned to maintain the said
equipment.

2.9. WORK EXPERIENCE (EXHIBIT I)

2.9.1 On Exhibit I list all the In-Plant contracts awarded to your company as a
primary contractor, for the last ten (10) years. Contractors must complete
all the applicable information requested under Exhibit "I". Use a separate
sheet for each contract.

2.9.2 As Exhibit I-1 list all the In-Plant General contracts awarded to your
company as a sub-contractor, for the last ten (10) years. Contractors
must complete all the applicable information requested under Exhibit "I".
Use a separate sheet for each contract.

2.9.3 Include only experience related to In-Plant work such as In-Plant piping,
underground utilities and installation of pumps and compressors, etc. DO
NOT list work experiences in 2.9.1 and 2.9.2 above that fall outside the
parameters of In-Plant Work as such information will not be considered in
the evaluation of your company for inclusion on this Bid Slate.

2.9.4 Provide back-up documents for non SAUDI ARAMCO Contracts listed
above. Enclose for each Contract, a copy (ies) of page(s) which shows
the Contract No., Title, Signature pages, Scope of Work, Duration and
Contract Value.

2.10. LOSS PREVENTION (EXHIBIT J)

Provide your own Exhibit "J" by providing information of your company safety
record for the last three (3) completed contracts.

2.11. QUALITY ASSURANCE/QUALITY CONTROL (EXHIBIT K)

2.11.1 As Exhibit "K" (in your own format) provide Saudi Aramco with a
departmental organization chart of your company's Quality
Assurance/Quality Control group. The chart must show the relationship
between this department and the company's management structure, as
well as identify the job titles and names of employees presently
responsible for quality control in your company.

2.11.2 As Exhibit "K-1", provide Saudi Aramco with a brief description or outline,
of your company's Quality Assurance/Quality Control program.

2.12. SAUDIZATION (EXHIBIT L)

As a separate attachment, identify the key management positions in your


companys organization chart that are presently occupied by Saudis and verify
their years of work experience and what University they graduated from.
Contractor must provide comprehensive information for this category with all
back-up information inclusive of one resume for each management position that
is currently Saudized and the following:

2.12.1 What is the percentage of the Saudi Arab workforce in your company?

2.12.2 Provide a list of Saudi Arab employees (with the duty joining date), who
are continuous in services with your Company over five (5) years.

2.12.3 Provide copy of latest labor office certificate of compliance to Royal


Decree 50.

2.13. CONTRACTOR EMPLOYEES AFFAIRS (EXHIBIT M)

As a separate attachment, provide a detailed outline of your Employees affairs as


follows:

2.13.1 When do you pay your employees salaries?

2.13.2 How often vacations are granted to employees of your company?

2.13.3 In what kind of transportation employees of your company are


transported?

2.13.4 Detail the accommodation of your employees such as type, size and
services provided.

2. Provide a brief description of how your company proposes to furnish accommodations for its
employees working on Saudi Aramco's In-Plant General Projects. Be sure to address areas
such as: transportation, catering, medical etc.

Our Company shall manage to get a camp on lease where all facilities like messing, transportation

Recreation and dispensary with a doctor shall be provided to all employees as per their position

and responsibility in the company. For transporting the staff we provide Car, Pick up (single &

double cabs), Van and bus as per category of the staff. Similarly good hygienic and healthy food

is provided three times a day. Lunch is normally provided at work site and the staff consumes it

at designated place for eating, usually a MESS HALL is arranged at site for this purpose. Eating

shall not be allowed at work locations. For site, first aid facility shall be arranged while in case of

emergency, the case shall be referred to a nearby hospital and suitable transport shall be

available at site during working hours (or overtime if required). Medical facility in the camp area

shall be provided for minor illness and routine physical disorders like flue, headache, muscular

fatigue, minor cut or injury and digestive disorders etc. and any other shall be referred to hospital

EQUIPMENT

Describe Companys equipment, maintenance and storage facilities, including locations


(with maps) and size of each facility plus the number and occupations of employees
assigned at each location.

The Company Winteco Premier Co. (WPC) has acquired a Laydown area of about 10,000 m2
on lease for a period of three years (extendable on mutual agreement) starting June 2011, in
Tuffaih near Askan of Ministry of Interior, Jubail.
This area is used for offloading and storage of project material and equipment when their direct
shifting at project site is not feasible. The location is selected near to the project site for
convenient transportation. At present, this area is being used for the construction support of 12
no. of Process and Storage tanks at SAMCo Acrylic Acid project at TASNEE petrochemicals
Complex Jubail.

Four persons (2x Equipment maintenance mechanics and 2x electricians) are deployed for
routine maintenance and upkeep of equipment and machines at laydown area as well as project
site. Any major repair or overhaul is conducted through hiring maintenance services from our
affiliate firms.

Contractor Pre-Qualification Safety Evaluation


Inquiry and Submittal Contents

Provide the following evidence to assist Saudi Aramco in evaluating the effectiveness of your
companys safety program. Evidence includes both: (1) written statements of what your
company intends to do (e.g., in your corporate safety program), and (2) samples of recent after-
the-fact actual records (at least 5 records over a time period of at least six months) to verify
proper implementation (e.g., signed attendance sheets, meeting minutes, training records,
certificates, completed checklists).

Written programs are to specify: (1) the responsibilities of involved personnel, (2) minimum
requirements/procedures for successful implementation of the program, (3) training on the
program to be provided to personnel responsible for implementing the program, and (4)
recordkeeping (documentation/forms to be maintained).

All submitted documents and records are to be in English and be submitted in the order listed
below, with tabs clearly separating each section.

WORK EXPERIENCE Submit your companys: (1) full name, (2) mail address, (3)
commercial registration number, (4) Saudi Aramco vendor number, and (5) list of
specific work disciplines/crafts for which your company is seeking to be approved to

statement) that is signed by your companys top management (e.g., CEO, Owner).

personnel responsible for implementing the program, and (4) recordkeeping
(documentation/forms to be maintained).
2. List of topics covered by your new employee safety orientation program, which are
applicable to the hazards of your companys operations and the employees type of work.

C. Training/Certification
Provide at least the following evidence that your companys employees receive safety
training, job skills/craft competency training, and required certifications:

1. Written safety training program that includes: (1) a description of the basic safety training
courses (e.g., first aid, fire safety, hazard recognition, confined space safety, H 2S safety,
driving safety) provided to your companys employees, (2) how these safety training
courses are delivered (e.g., in-house, third-party), and (3) which of these courses are
required for each type of job (e.g., documented in a safety training matrix or safety
training plan).
2. Recent records (at least 5) to verify proper implementation of your companys safety
training program (e.g., signed attendance sheets, employee training records).
3. Written job skills/craft competency training program that includes: (1) a description of
the specific job skills/craft training courses required for each type of job (e.g., HVAC
technician, plumber, pipefitter, scaffolder, welder) that are specific to their work for Saudi
Aramco, (2) how these job skills/craft training courses are delivered (e.g., in-house and/or
third-party), and (3) refresher training frequency.
4. Recent records (at least 5) to verify proper implementation of your companys job
skills/craft training program (e.g., signed attendance sheets, employee training records).
5. List of jobs performed by your companys employees that require Saudi Aramco-
approved certification (e.g., scaffold supervisor/inspector, crane/heavy equipment

responsibilities, requirements/procedures, training, and recordkeeping) that includes
safety performance.
2. Recent records to verify that employee and supervisory performance appraisals, which
include safety performance, are conducted.

Provide the following information to assist Saudi Aramco in evaluating your companys
procedures that specifically address the following for each separate job.

A. Hazards Understood
Provide at least the following evidence of your companys procedures to identify and mitigate
safety and health hazards your employees may encounter while performing contracted work:

1. Written program (specifying responsibilities, requirements/procedures, training, and


recordkeeping) for identifying, evaluating, and controlling workplace hazards (e.g.,
Hazard Identification Plan [HIP], Job Safety Analysis [JSA]).
2. Recent records from previous jobs to verify that your company has actually implemented
procedures to (1) identify, (2) evaluate, and (3) control workplace hazards.

B. Personal Protective Equipment (PPE)


Provide at least the following evidence of your companys procedures for assessing PPE
needs, the specified quality/standards (e.g., ANSI) of each type of PPE purchased, and
employee training in the proper use of PPE:

1. Written personal protective equipment (PPE) program (specifying responsibilities,


requirements/procedures, training, and recordkeeping), as well as a list of the specified
standards (e.g., ANSI #) for each type of PPE purchased by your company.
2. Records (e.g., purchase orders/invoices, copies of PPE labels/markings, manufacturers
literature) that each type of PPE purchased by your company actually meets the above
