This chapter discusses the mechanisms that are believed to contribute to optic nerve
injury and retinal ganglion cell (RGC) loss in glaucoma. Althought the precise cause of RGC
death in glaucoma is unknown, several mechanisms have been proposed. These include:
mechanical compressions due to elevated intraocular pressure (IOP), neurotrophic factor
deprivation, excitotoxicity, ischemia, hypoxia, and oxidative stress.
A current view is that RGC degeneration occurs in two stages. First, an insult occurs
at the level of axons in the optic nerve leading to death of damaged RGCs. Second, a self-
perpetuating cascade of events that includes the release of harmful stimuli from dying
neurons leads to progressive loss of neighboring RGCs that were not damaged by the initial
injury (secondary degeneration). This model of biphasic loss of RGCs involving the
progressive demise of otherwise healthy neurons may explain why a significant proportion
of glaucoma patients continue to experience visual loss in spite of responding well to IOP
lowering medications.
Axonal Injury
There is evidence that the initial site of injury in glaucoma is in RGC axons within the
optic nerve. Two mechanisms have traditionally been offered to explain axon degeration in
the central nervous system. In the dying back hypothesis, axon degeneration (caused by a
chronic stress) begins at the distal ends of the injured axon and spreads in a retrograde
fashion. In the focal lesion hypothesis (eg, wallerian degeneration), one or more focal
injuries, not neccessarily transecting the axon, trigger degeneration of the entire axon distal to
the lesions.
Both dying back and focal lesion axonal degeneration have been documented in a
wide range of neurological disorders, and sometimes both coexist in single desease. It is
likely that focal axonal injury plays a major role in the development of RGC pathology in
glaucoma. RGC axons are more vulnerable as they pass through the pores of the sieve-like
lamina cribosa at the optic nerve head. Elevated IOP distorts the shape of dimension of
lamina cribosa pores resulting in compression RGC axons in specific locations. This
localized injury may result in discruption of axonal transport, blocking passage of
fundamental molecules required for neuronal manitanance and function or leading to vascular
ischemia and hypoxia.
Neurotrophic factor deprivation
During development of the nervous system, neurons are produced in exces and then
eliminated by an intrinsic cell death programs known as apoptosis. This self-destructive
program is activated in all neurons, and only those that receive the appropriate throphic
factors secreted by their targets can escape death. This process forms the basic of the
neurotrophic factors that are required for survival and differentiation. It is now clear that
neurotrophic factors act by actively suppressing the intrinsic apoptotic process and by
activating survival signals.
The neurotrophins, brain-derived neurotrophic factor (BDNF), nerve growth factor
(NGF), neurotrophin-3 (NT-3) and neurotrophin-4 (NT-4), are the best-characterized family
of neurotrophic factors. There is strong direct evidence that deprivation of the BDNF and
NGF secreted by RGC targets in the brain leads to apoptotic death of developing RGCs.
This is consistent with recent findings that neurotrophins can initiate signals that are
sent backwards through long axons and keep the cells alive.
The role of these endogenouse factors in the maintenance of adult RGCs is less clear.
It has been proposed that glaaucomatous damage leads to blockade of axonal transport,
leading in turn to neurotrophin deprivation and neural death. Indirect evidence for this is
provided by the observation that experimental IOP increase impairs transport of BDNF and
its receptor from RGC targets to soma in the retina.
However, it has also been proposed that target-derived neurotrophins have little or no
influence on the survival of mature RGCs; rather, it is through taht local sources of
neurotrophins wwithin the retina play a prominent role. Evidence of this premise comes from
studies in which RGC target tissue was ablated in adult animals and no RGC loss was
detected for several months after the lesion. Importantly, however, retrograde transport of
neurotrophic factors continues along adult RGC axons. Based on this, it has been proposed
that target-derived factors may act to compensate for a deficiency at the level of local trophic
support. Thus, it is possible that neurotrophin deficiency, either from the target or the retina,
may play a role in RGC death in galucoma. Future experiments should focus on resolving this
important issue.
Induction of apoptosis
As noted, apoptosis is an evolutionarily concerved mechanism of programmed cell
self-destruction. The word apoptosis has a Greek root that literally means falling (ptosis)
from a trees. Apoptosis is seen in many neurological diseases, including glaucoma. While a
certain level of apoptosis is associated with normal tissue homeostasis and cellular turnover,
excessive and uncontrolled aapoptosis is a hallmark of neurodegeneration. There is now little
doubt that apoptosis is a cemtral mechanism of RGC death in glaucoma. Neurons undergoing
apoptosis are characterized by a distinct morphology with shrinkage of the cell body,
chromatin condensation, and intracellular fragmentation.
Apoptosis involves the activation of a genetic program and de novo systhesis of
proteins that play crucial roles during the process. Four distinct phases have been identified in
the apoptotic process: activation, signal integration, execution, and clearance.
Activation of apoptosis occurs when a extracellular or intracellular event signals a
new condition for the cell. For example, trophic factor deprivation (extracellular) or oxidative
damage that results in the generation of free radical species (intracellular), among many other
stimuli could trigger apoptosis of RGCs in glaucoma.
Signal integration consists of the regulation of cellular events that will take place once
the apoptotic program has been activated, this program will ultimately determine whether the
cell lives or dies. An important class of intracellular regulators of apoptosis, the Bcl-2 family
of proteins, has been evolutionarily concerved from worms to humans.
Several Bcl-2 family members have been identified in mammals and some of these in
hibit apoptosis (eg, Bcl-2, Bcl-X1) and others promote it (eg, Bax, Bad, and Bid). The Bcl-2
proteins regulate apoptosis in several ways, but because they have a hydrophobic tail that
enables them to bind to the outside surfaces of motochondria, they function mainly on these
organelles. For example, Bcl-2 can inhibit the release of cytochrome C from the mitochondria
and thus prevent the consequent activation of proteases involved in dismantling the cell.
Consistent with this Bcl-X1 overexpression has been shown to protect RGCs following optic
nerve transaction. Bax, on the ether hand, acts as a proapoptotic molecule by binding to
mitochondria, where it can increase membrane permeability and cytochrome C release. A
recent study demonstrated taht RGCs are preserved in Bax-deficient DBA/2J mice (which
serve as a model of inherited glaucoma), indicating that Bax is required for RGC death in this
model. Iterestingly, however, the same study showed that lack of Bax did not prevent axonal
degeneration.
Caspases are a family of proteases that execute the dismantling and demolition of
apoptotic cells. they are synthesized as large inactive precursors (procaspases) and become
active ones they are cleaved, usually by another caspase. Hence, these proteases have been
classified into two broad categories; initiator caspases (eg, caspases 1, 2, 4, 5, 8, 9, and 10)
which activate other caspases and effector caspases (eg, caspases 3, 6, 7, and 14) which
cleave specific substrates involved in the actual annihilation of the cell. Inhibition of effector
caspase 3 has been shown to be neuroprotective.
The final stage of apoptosis is clearance of the cellular debris. Unlike neurotic cells,
apoptotic cells shrink and are phagocytosed by neighboring or immune cells, before there is
any leakage of cellular contents, thus avoiding inflammatory responses.
