AFFIDAVIT OF DISCREPANCY

I, MANUEL DINGDING, of legal age, Filipino, married, and a resident of Lagtang,

Talsiay City, Cebu, Philippines, having been duly sworn in accordance with law, hereby
depose and state:

1. That in my Certificate of Live Birth as recorded in the Office of the Civil

Registrar of Alcoy, Cebu, Philippines, I have no middle name and it was
erroneously written as JIMENEZ instead of no middle name in my Office of the
Senior Citizens Affairs (OSCA) City of Talisay, Cebu Identification Card with
Control Number 17706;

2. That I have no middle name as appearing in the Certificate of Live Birth;

3. That I am executing this Affidavit of Discrepancy to attest to the truth of the

foregoing facts and to explain the discrepancy in my Office of the Senior
Citizens Affairs, City of Talisay, Cebu, as regards that I have no middle name.
