I.

Objective:
A. General
To be able to increase our knowledge with the procedures and routines done within the hospital,
as well as incorporate new concepts learned from the classroom to the actual duty area.
B. Specific
To identify learning opportunities and to be able to increase knowledge as well as skills on routine
hospital procedures especially in the male ward.

II. Observation
Self:
there are still a lot of things i need to learn and improve. my writing, should be deciphered before
anyone could read and finally understand what it says. the slower i write the smaller it gets, the
faster i write it becomes unreadable. i hope with in this rotation i could finally at least improve it
somehow. btw, i need to build more self-confidence to what i know, so i could be able to do the
task at hand. hina kc ng loob ko minsan..hmm

Groupmates:
there are a lot of instances when i cant be of equal footing when it comes to my groupmates like
when it comes to designing things, i just don't have any talent at all compare to them..so artistic
and talented.

Clinical Instructor:
has a lot of patience especially when it comes to our groupmate who keeps bugging her with
inquiries (ahem). yet in spite of it i learned a lot in just a few days.

III. Learning
every time am assigned to a patient i cant avoid seeing myself in their position. i always reminds
me of my time in the hospital not as a student nurse but as a patient. with this notion, it helps me
to understand them more, have more patience, and vigilance. however, i should also be careful
not to think ahead of things, which sometimes is to drastic. i should first exhaust everything a
nurse could do before entertaining collaborative interventions.

Observations:
a. Self - I am doing all my best to improve more my potentials and skills in handling different pts.
and especially to prove that I could be a responsible "NURSE" in the future.
b. Group mates - Some of my group mates are moody but inspite of that I'm very lucky to have
them as my groupmates because with them, I know that I am not alone. They are very responsible
and willing to help each other, sharing what they know and what they have learned.
c. C.I - Some students, especially those who haven't experienced under her teachings says thatshe
is strict and "terror" teacher but how can they say that she is terror if they really dont know who is
Ma'am Nhelia, the way she teach, she acts, talks and others. Ma'am Nhelia is one of a kind
instructor, jolly and intelligent. She let her students learn from their mistakes and I admire the
way she discipline her students.

Learnings:
I've learned to be more flexible in handling patients especially when I experienced to handle a
patient with altered mental status at Male ward. It is my pleasure that Ma'am Nhelia trusts my
capabilities and potentials to handle like that kind of patient and because of that trust I will do all
my best to prove that I deserve to be trusted.
Handling pt. with altered mental status is not that too easy task, you have to be more patience and
understandable. Especially, you have to be sensitive enough to the needs of your patient.

A. Objectives
General: to enhance our knowledge regarding the different diseases in the hospital
Specific: to experience and to perform care to the patient

B. Observation
1. Self: i feel more confident now because with the help of people around me, we are talking all the
consequences to face the future. to face all the challenges in our duty. I always believe in myself,
self-confidence is one of my treasure, another chapter another experience, education had
enriched me so much gift to make me a better person to share all of this to people that i meet.
2. Groupmates: to my groupmates we enjoy the duty maybe because of the bonding, sharing of
ideas, and especially helping each other.
3. Clinical Instructor: to my clinical instructor, ma'am thank you for the experience and for
making us a better and responsible student nurses. those demerits are one of your key, to make us
to be careful in everything that we do. i must be thankful to god, if not for those low points, we
would not appreciate the high points better. ma'am Nhelia is one of my CI that i really like
because of her own good way and being so approachable in every inquiries.
C. Learning
Iv learned a lot in 3 days of our duty because we all are taking care of our patients. i obviously
realized that the path to success is hard but through courage and determination, we can get
through everything. I've done some mistakes in medication, from it I've learned to focus on what
the CI is saying/asking. and will not give any answer unless I'm really sure that its the right one.
ma'am thanks for sharing, giving, and understanding us and for the quality time you have spent in
our duty.

Objectives
a. General: To be able to be familiarized again to the routine in the Male ward and to learn more
and develop my skills in rendering nursing interventions.
b. Specific: To render good care to my patients during our shift and to gain more knowledge and
gather information about their disease.

Observations:
a. Self: I feel nervous because it is male ward again. Expecting that it is the toxic ward but still
doing my part in my best way I can be.
b. Group mates: as our shift started, I observed that all are busy most especially upon carrying out
the Doctors orders. Im glad because my group mates were very cooperative.
c. Clinical Instructor: Our instructor became more effective because she is discussing about our
patients illnesses during our free time. She does not let us commit mistakes or errors especially in
giving medications.

Learning: It is not learning, instead, a lesson. My phone was confiscated during the second day of
our rotation. I feel sad that day. And the saddest thing is, I am not the one who is using my phone
when it was confiscated huhuhu. I miss my phone a lot. Because of that happen, I cant even
concentrate in handling my patient because I always think of my phone and thinking what would
I tell my mother if she ask where is my phone. Hmp, well well well, it is a part of being pazaway
sometimes so I admit its all my fault.

General Objective: In order to render care to patients in the male ward and increase more

Specific Objective: In order to share and provide care to patient in the surgery ward and help in
his quick recovery.

Observations:

A. SElf - It is not new to me to have duty in the male ward but it good to see myself enjoying every
minute of my duty. I am very thankful for the incessant supervision and guidance by our clinical
instructor. Im learning and still want to improve my skills and knowledge in the clinical area.

B. Groupmates - They were very busy in ther own patients, and they were doing very well.
However, we still have time to chat and share our experiences with our patients. We share
knowledge and try to help our groupmates whenever needed.

C. Clinical Instructor - Shes very knowledgeable in the clinical area and shes also a skilled nurse.
She nver fails to remind us and guide us on proper clinical care to our patients. She imparts
knowledge everytime and shes fun to be with. Shes approachable and easy to get along with her.

Learnings:

Every patient that I care, theres always the experiences that I will nevr forget. And it always
reminds me to do better and to do the best I can next time. I learned to be more patient and
humble whoever my patient is, whatever my patient has, and whatever race, value or attidute he
shows toward me. It is my duty to be caring, and that should be reflected everytime. I learned to
be more careful in observing the 11 rights in the clinical area. And most of all, I learned to build
harmonious relationship with my groupmates, CI, staffs and the patient.

II. Observation
Self: i'm slightly disappointed with myself coz i wasn't able to do well my tasks for the week. i
wasn't able to finish my assignment completely and had more demerits. also,i felt sad that i was
not trusted to handle the suicidal patient. i just hope i could catch up next week.;)
Group mates: i love my group mates because they are all approachable, cooperative, artistic and
fun-to-be-with,even if some of them are moody. uhm, i just want to thank ms.pagaduan and
ms.luis for their support and company for me all the time.
C.I.: ma'am nhelia is sooo unique! she's cool and fun-to-be-with c.i yet she teaches well. she's very
approachable but,.she's not considerate in terms of her "favorite" demerit system!haha :)

Learnings: even though i had only one patient for the week,and with the dx of AGE,i still got
learnings from this week. i made mistakes and from it,i gained more. i should develop more in
charting and respect others by calling them with sir or ma'am. i was able to practice more in
preparing and administering medications,assessing the pt, getting the history,carrying out new
orders and etc.

Learning Feedback Diary

I: General objective

> To develop more our knowledge and skills in promoting nursing health care in handling
different cases of patients.

II: Specific objective

> To render health care services in our respective patients.

III: OBSERVATIONS

a.)SELF: As a fourth year student,I want to gain more my knowledge & skills with regards in
rendering health care services in handling different patients.Despite of being toxic in the ward,I
worked with a lot of patience in order to promote good nursing care.

b.)GROUPMATES:My groupmates were responsible enough in caring of their patient but


sometimes they forgot to act like a professional in a way that calling somebody with their names
that's why in the first week of our rotation almost of them earned a lot of demerits just because of
that, but behind that they worked with cooperation,all of them are approachable and willing to
help each other.
c.) C.I : Our clinical instructor is very strict when it comes to charting,she always says that the
patient's chart is confidential and we have to be careful in documentation.Every time we created
an error there is a corresponding demerits.In that way, she wanted us to become responsible and
conscious in what we are doing because as you make an error the life of the patient will become in
danger especially in administering medications that's why she always accompanying us.She gives
informations that helps us on how to become a responsible one in assessing and caring of our
patient.a

LEARNINGS

Every time I assigned in one patient a lot of things which I had learn, first is on how to deal with
the patient how to gain trust and cooperation with the patient and the S.O ,second is on
administering of drugs, another one is on how to render care in a surgery patient,one thing more
is on how to make an assessment that will be documented in the form of SOAPIER.
I learned also on how to manage my time, on how to deal with the staff,our clinical
instructors,with my groupmates professionally.
One lesson that i had learn is to make sure that all latest doctor's order will be carried out before
approaching the C.I.in order to avoid earning a lot of stars.

I. Obejective
General Objective: to provide quality care to orthopedic patient
Specific objective: do basic nursing skills like v/s, medication, provide health teaching, maintain
immobilization and skin integrity, hygiene and sometimes "stimulation" - shyr

II. Observations
a. self: i was able to exercise basic nursing skills in orthopedic patient, do basic assessment skills,
required and provide proper care and management by maintaining proper body alignment,
providing pain management and giving health teaching.

b. Groupmates: we were not so "toxic" when it comes to patients but had mistakes which was
forgivable at least we never injured anyone. plus i have a groupmate who is good in providing
stimulation.hehehe

c. Clinical Instructor: never tired in reminding us our faults, and rewards good performances with
praise.

III. learnings
for this week i was trying to be "artistic" hehehe hopefully ma'am nhelia appreciates it.
- orthopedic patients require delicate and more involved care because you have to be careful with
their affected parts or else you would hear is "ouch"

Observations
A. Self: I earned lot of demerits this week, despite my cough and colds, i still done my part in
rendering care to my patient
B. Groupmates: there were very curious and aggressive. they are all intelligent, thatts one thing
challenges me.
C. Clinical Instructor: shes very nice instructress. she does not neglect the group. she initiates us
in every procedure. thank you ma'am for imparting us you knowledge.

Learnings
be very careful enough in rendering care to our patient. we have to have self-confidence in
oursleves. have presence of mind always
