CONTRACT OF WORK

This contract of work is entered into by and between:

INNOVATIVE DESIGNS SOLUTIONS located at _______________________________________ represented by

its Manager, ______________________, Filipino citizen, of legal age, single with residence address the same
above with TIN No._______________ herein referred to as the Contractor, and

NAME OF SCHOOL located at_____________________________________________________, represented by

its Principal ________________________________, Filipino citizen, of legal age.

I. SCOPE OF WORKS. That, both parties hereby agree to design/layout and print Yearbook of the
students for the School Year 20__-20__. Below are the detailed specifications of works to be done.

100 pcs AY 16-17 Yearbook Album PHP ________

200 pages
Using 9 x 11 C2S quality paper
At monochrome prints,
Paper bound with 230 gsm
Colored coverings
At P _____/Album

II. CONTRACT PRICE AND PAYMENT

a. That the total service contract price shall be _____________________________That 50% down
payment is required upon signing and the balance shall be paid upon the delivery
date.

III. TERMS AND CONDITIONS

a. The Contractor shall submit in a CD the final layout of the yearbook as part of their completion
report.
b. The Contractor shall receive an actual payment based on the actual number of yearbook printed.
c. The Contractor shall complete the project (submission of materials from the school, lay-outing
of materials, proofreading, finalization and printing) for 90 working days, a day after receiving
the down payment.

IV. The parties to this agreement hereby agree to the covenant contained herein and in witness hereof,
affix their signatures this 19th day of September 20___ at _________________________.
